摘要:
目的:观察采用股骨头坏死愈胶囊联合头颈部开窗打压植骨手术治疗ARCO Ⅱ、Ⅲa期股骨头坏死的临床疗效及安全性。方法:将2019年5月至2020年7月收治的股骨头坏死患者58例(70髋)分为服药联合手术治疗组(观察组)及单纯手术治疗组(对照组)。观察组行股骨头坏死愈胶囊联合头颈部开窗打压植骨术,对照组仅行头颈部开窗打压植骨术。观察两组治疗前、术后半年及截至2021年11月末次随访时的Harris评分、视觉模拟评分法(VAS)评分,并用X线片评价治疗前后股骨头形态及植骨情况。结果:两组随访时间12～18个月,平均14个月,两组患者均无感染、下肢深静脉血栓等并发症。末次随访时与治疗前相比,两组患者的Harris评分较术前均有大幅度提高,VAS评分较术前有所降低,差异均有统计学意义(P<0.05),且末次随访时,观察组患者的Harris评分、VAS评分及影像学表现均优于对照组,差异均有统计学意义(P<0.05)。结论:股骨头坏死愈胶囊联合头颈部开窗打压植骨术治疗ARCO Ⅱ、Ⅲa期股骨头坏死的中短期疗效好,可作为治疗早中期股骨头坏死的推荐方法。
Abstract:
Objective:To observe the clinical efficacy and safety of Gugutou Huaisiyu capsules combined with femoral head neck fenestration and bone grafting in the treatment of ARCO Ⅱ and Ⅲa femoral head necrosis.Methods:A total of 58 patients(70 hips)with femoral head necrosis admitted from May 2019 to July 2020 were divided into medicine combined with surgical treatment group(observation group)and surgical treatment group(control group). The observation group received Gugutou Huaisiyu capsules combined with femoral head neck fenestration and bone grafting. The observation group received femoral head neck fenestration and bone grafting only. Harris scores and visual analogue scale(VAS)scores were observed before treatment, half a year after surgery and at the last follow-up. The shape of femoral head and bone grafting were evaluated by X-ray before and after treatment.Results:The follow-up time of the two groups was 12 to 18 months with 14 months on average. There was no complications such as infection and deep venous thrombosis of lower limbs in both groups. At the last follow-up, Harris scores, VAS scores and imaging performance of the observation group were better than those of the control group, while Harris scores, VAS scores and imaging performance of the observation group were significantly higher than those of the control group. The differences were statistically significant(P<0.05).Conclusion:Gugutou Huaisiyu capsules combined with femoral head neck fenestration and bone grafting has good efficacy in the short and medium term for femoral head necrosis with ARCO Ⅱ and Ⅲ. It can be used as a recommended method for the treatment of early and middle stage femoral head necrosis.
