摘要:
目的:观察自体带骨膜髂骨栓移植治疗HeppleⅤ型距骨内穹隆骨软骨损伤疗效。方法:分析自2019年8月至2020年10月收治的39例HeppleⅤ型距骨内穹隆骨软骨损伤患者临床资料。术后观察患者并发症发生率、末次随访时踝与后足评分量表(AOFAS)评分、疼痛视觉模拟评分法(VAS)评分及优良率。结果:所有患者术后均无手术相关并发症发生; 术后12个月随访时,VAS评分均较术前显著降低,差异有统计学意义(P<0.001); AOFAS评分均较术前显著提高,差异有统计学意义(P<0.001); 疗效优10例,良22例,可7例,优良率为82%。结论:自体带骨膜髂骨栓移植治疗HeppleⅤ型距骨内穹隆骨软骨损伤疗效确切,并发症少,手术操作简单,易于基层医院推广应用。
Abstract:
Objective:To observe the efficacy of autologous periosteal iliac bone plug transplantation on the treatment of Hepple type Ⅴ intra talar dome osteochondral injury. Methods:A trial was conducted to analyze the clinical data of 39 patients with Hepple type Ⅴ intra talar fornix osteochondral injury from August 2019 to October 2020. The incidence of complications, ankle and hindfoot score(AOFAS), visual analogue scale(VAS)scores, excellent and good rate were compared between those before operation and the last follow-up. Results: There was no postoperative complications in group. At the follow-up of 12 months after operation, the VAS score of the group was significantly lower than that before operation(P<0.001). The AOFAS score was significantly higher than that before operation(P<0.001). In the group, 10 cases were excellent,22 cases were good and 7 cases were fair, and the excellent and good rate was 82%. Conclusion:The autologous periosteal iliac bone plug transplantation is effective on the treatment of Hepple type Ⅴ intra talar fornix osteochondral injury is effective, with few complications and simple operation, which is easy to be popularized in grass-roots hospitals.
