摘要:
目的:分析比较股骨近端防旋髓内钉(PFNA)与联合拉力交锁髓内钉(InterTan)治疗不稳定性转子间骨折的临床效果。方法:收集2010年7月至2014年11月就诊并采用PFNA或InterTan治疗的不稳定性转子间骨折患者95例,将病例分为PFNA组和InterTan组,记录两组患者手术时间、术中出血量、术中透视次数,术后随访统计患者骨折愈合时间、髋关节功能评分(Harris评分)、髋关节功能优良率、术后并发症情况等。结果:两组患者均获得1年以上随访,随访时间(16.1±5.9)个月,两组患者术中出血量、术中透视次数、骨折愈合时间、术后优良率、并发症率等差异均无统计学意义(P>0.05); PFNA组患者手术时间短于InterTan组患者,InterTan组患者术后末次随访Harris评分高于PFNA组患者,差异均有统计学意义(P<0.05)。结论:PFNA和InterTan均适用不稳定性转子间骨折,术中出血量均较少,并发症率低; PFNA手术时间较短,但其稳定性较差; InterTan手术时间较长,但稳定性高,术后髋关节功能评分优于PFNA.
Abstract:
Objective:To analyze and compare the clinical results of the treatment of unstable intertrochanteric fractures with proximal femoral nail antirotation(PFNA)and InterTan. Method:From July 2010 to November 2014, 95 patients with unstable intertrochanteric fractures treated with PFNA or InterTan were divided into PFNA group and InterTan group. In two groups, the operation time, blood loss, intraoperative X-ray times, the fracture healing time, Harris score, the excellent and good rate of hip joint function and postoperative complications were recorded. Results:The two groups were followed up for more than 1,(16.1±5.9)months. There was no significant difference in blood loss, intraoperative X-ray times, the fracture healing time, the excellent and good rate of hip joint function and postoperative complications between the two groups(P>0.05). The operation time of PFNA group was shorter than that of InterTan group, and the Harris score of InterTan group was higher than that of group PFNA after operation, the difference was statistically significant(P<0.05).Conclusion:Both PFNA and InterTan operation were suitable for unstable intertrochanteric fractures, with less blood loss and low complication rate. InterTan operation time is longer than PFNA, but the stability is higher, with better hip function.
