摘要:
目的:探究补肾活血汤联合等离子射频消融术治疗椎间盘源性腰痛的临床疗效及对炎性因子的影响。方法:选取椎间盘源性腰痛肾虚血瘀证且符合纳入标准的患者共60例,分为观察组和对照组各30例。两组患者行低温等离子射频消融术治疗,观察组在此基础上口服加味补肾活血汤。对比两组患者视觉模拟量表(VAS)评分、Oswestry功能障碍指数(ODI)评分、中医证候评分、改良MacNab临床疗效评分、血清细胞因子水平,所有数据均采用GraphPad Prism10.1.2软件进行分析。结果:两组患者的VAS评分较治疗前均明显降低,差异有统计学意义(P<0.05); 治疗后3个月时,观察组的VAS评分前后差异相较于对照组更为明显,差异有统计学意义(P<0.05)。治疗后各时间点两组患者的ODI评分较治疗前均明显降低,差异有统计学意义(P<0.05),两组间差异无统计学意义(P>0.05)。治疗后两组患者的中医证候评分较治疗前均明显降低,差异有统计学意义(P<0.05),观察组较对照组下降明显,差异无统计学意义(P>0.05)。观察组改良MacNab临床疗效评分优秀率为70.00%,对照组的优秀率为43.33%,两组优良率差异有统计学意义(P<0.05)。观察组与对照组患者的致炎因子(IL-1β、IL-6、IL-8、TNF-α)较治疗前均明显降低,差异有统计学意义(P<0.05),且两组间差异有统计学意义(P<0.05)。抗炎因子IL-4在术后均明显升高,差异有统计学意义(P<0.05),且两组间差异有统计学意义(P<0.05)。结论:补肾活血汤联合等离子射频消融术治疗椎间盘源性腰痛疗效显著,能够迅速、明显地改善腰痛症状,减低炎症反应。
Abstract:
Objective:To explore the clinical efficacy of Bushen Huoxue decoction combined with plasma radiofrequency ablation on discogenic low back pain and its effect on inflammatory factors.Methods:A total of 60 patients with discogenic low back pain and Shenxu Xueyu syndrome who met the inclusion criteria were selected and divided into observation group and control group with 30 cases in each group.Both groups of patients underwent low-temperature plasma radiofrequency ablation,and the observation group took modified Bushen Huoxue decoction on this basis.The VAS scores,ODI scores,TCM syndrome scores,modified MacNab clinical efficacy evaluation criteria,and serum cytokine levels of the two groups were compared,and all data were analyzed using GraphPad Prism10.1.2 software.Results:The VAS scores of the two groups of patients were significantly lower than those before treatment(P<0.05).At 3 months after treatment,the difference in VAS scores before and after treatment in the observation group was more obvious than that in the control group(P<0.05).The ODI scores of the two groups of patients at each time point after treatment were significantly lower than those before treatment(P<0.05),and there was no statistically significant difference between the two groups(P>0.05).After treatment,the TCM syndrome scores of the two groups of patients were significantly reduced compared with those before treatment(P<0.05),and the decline in the observation group was more obvious than that in the control group(P>0.05).The modified MacNab clinical efficacy evaluation showed that the excellent rate of the observation group was 70.00%,the excellent rate of the control group was 43.33%,and the difference in the excellent rate between the two groups was statistically significant(P<0.05).The pro-inflammatory factors(IL-1β,IL-6,IL-8,TNF-α)of the observation group and the control group were significantly reduced compared with those before treatment(P<0.05),and there was significant difference between the two groups(P<0.05).The anti-inflammatory factor IL-4 increased significantly after surgery(P<0.05),and there was significant difference between the two groups(P<0.05).Conclusion:Bushen Huoxue decoction combined with plasma radiofrequency ablation is effective in the treatment of discogenic low back pain,which can quickly and significantly improve the symptoms of low back pain and reduce inflammatory response.
