摘要:
目的:探讨白蛋白(Alb)与纤维蛋白原(FIB)比值(AFR)在老年髋部骨折后下肢深静脉血栓(DVT)形成中的诊断价值。方法:选取2019年2月至2021年5月收治的髋部骨折患者276例为研究对象,多因素Logistic回归分析影响老年髋部骨折后下肢深静脉血栓的风险因素,应用受试者工作特征(ROC)曲线分析白蛋白、纤维蛋白原及白蛋白与纤维蛋白原比值诊断老年髋部骨折后下肢深静脉血栓的价值。结果:与非深静脉血栓组相比,深静脉血栓组纤维蛋白原水平明显升高,白蛋白水平及白蛋白与纤维蛋白原比值明显降低,差异有统计学意义(P<0.05); 多因素Logistic回归分析结果显示,纤维蛋白原是危险因素,白蛋白及白蛋白与纤维蛋白原比值均是保护因素,差异有统计学意义(P<0.05); 受试者工作特征曲线显示,纤维蛋白原、白蛋白、白蛋白与纤维蛋白原比值对老年髋部骨折后下肢深静脉血栓诊断的曲线下方面积(AUC)分别为0.727,0.665和0.815,白蛋白与纤维蛋白原比值对老年髋部骨折后下肢深静脉血栓的诊断价值更高。结论:纤维蛋白原升高、白蛋白降低是老年髋部骨折患者并发下肢深静脉血栓的危险因素,白蛋白与纤维蛋白原比值诊断老年髋部骨折患者下肢深静脉血栓的价值较高。
Abstract:
Objective:To investigate the diagnostic value of albumin(Alb)to fibrinogen(FIB)ratio(AFR)in the formation of deep vein thrombosis(DVT)of lower extremity after hip fracture in elderly patients.Methods:276 patients with hip fracture from February 2019 to May 2021 were selected as the research objects,multivariate Logistic regression analysis was used to analyze the risk factors for lower extremities DVT after hip fracture in elderly,receiver operating characteristic(ROC)curve was used to analyze the value of Alb,FIB and AFR in the diagnosis of lower limb DVT in elderly patients with hip fracture.Results:FIB level in DVT group was significantly increased compared with non-DVT group,Alb level and AFR level were significantly decreased,and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that FIB was a risk factor,Alb and AFR were both protective factors,and the difference was statistically significant(P<0.05).ROC curve showed that the area under curve(AUC)of FIB,Alb and AFR for the diagnosis of lower limb DVT after hip fracture in elderly patients was 0.727,0.665 and 0.815,respectively,and AFR had a higher diagnostic value for lower limb DVT after hip fracture in elderly patients.Conclusion:Increased FIB and decreased Alb are risk factors for lower limb DVT in elderly patients with hip fracture,and the value of AFR in diagnosing lower limb DVT in elderly patients with hip fracture is relatively high.
