摘要:
目的:探讨冰硝散在加速无引流管膝关节置换围手术期康复过程中的临床疗效。方法:选取2016年7月至2019年5月收治的559例单侧TKA手术患者,采用芒硝、冰片外敷辅助消肿、止痛治疗(冰硝散组); 选取2013年6月至2015年6月收治的412例初次单侧TKA手术患者,采用常规的局部冰敷辅助消肿、止痛治疗(冰敷组)。两组均在术后治疗7 d,比较两组术前、术后疼痛情况,髌上10 cm、髌周、髌下10 cm周径,治疗满意度,术后7 d HSS评分。结果:两组患者基线资料比较,组间差异无统计学意义(P>0.05); 手术前1 d,两组疼痛、肿胀及关节功能评分比较,差异无统计学意义(P>0.05)。术后第1天和第2天两组VAS评分比较,差异无统计学意义(P=0.816,0.783); 术后第3天和第7天,两组VAS评分比较,差异有统计学意义(P=0.002,<0.001),冰硝散组低于冰敷组。患肢周径比较,在术后第3天和第7天髌上10 cm、髌周两组差异均有统计学意义(P<0.05); 术后第3天,髌下10 cm周径,两组比较差异无统计学意义(P>0.05); 术后第7天,髌下10 cm周径,两组比较差异无统计学意义(P=0.664)。术后第7天,两组治疗满意度及HSS评分比较,冰硝散组均高于冰敷组,差异有统计学意义(P<0.001,P=0.001)。结论:冰片配合芒硝外敷辅助治疗膝关节置换术后疼痛、肿胀疗效确切,患者满意度高,功能恢复较传统冰敷要迅速,值得临床推广应用。
Abstract:
Objective:To investigate the clinical efficacy of mirabilite and borneol powder external application on promoting rapid rehabilitation after total knee arthroplasty(TKA)without percutaneous drainage.Methods:Mirabilite and borneol powder group:559 eligible patients with unilateral TKA between July 2016 and May 2019 were treated with mirabilite and borneol powder external application to relieve swelling and pain.Ice compress group:412 patients with primary unilateral TKA between June 2013 to June 2015 were treated with conventional local ice compress to relieve swelling and pain.The preoperative and postoperative VAS scores,circumference of 10 cm above patella,10 cm around patella and 10 cm below patella and degree of satisfaction were recorded and compared respectively,and HSS scores at the 7th day after operation were compared between the two groups.Results:There were no significant differences between the two groups in the baseline data,VAS scores,limb circumference,HSS scores preoperatively(P>0.05).There was no significant difference in VAS scores between the two groups at the 1st and 2nd day after operation(P=0.816,0.783).The VAS scores of Mirabilite and borneol powder group at the 3rd,7th day after operation were lower than those of ice compress group,and the difference was statistically significant(P=0.002,<0.001).There were significant differences in the circumference of 10 cm above patella,10 cm around patella at the 3rd,7th day after operation and the circumference of 10 cm below patella at the 3rd day after operation(P<0.05),and there was no significant difference in the circumference of 10 cm below patella at the 7th day after operation between the two groups(P=0.664).The degree of satisfaction and HSS scores of Mirabilite and borneol powder group at the 7th day after operation were higher than those of ice compress group,the difference was statistically significant(P<0.001, P=0.001).Conclusion:External application of mirabilite and borneol powder is effective on the treatment of pain and swelling after TKA,with higher degree of satisfaction and rapid functional recovery compared with traditional ice compress,which is worthy of clinical promotion.
