摘要:
目的:观察六黄散外敷配合口服四妙散治疗急性痛风性关节炎湿热蕴结证的临床疗效。方法:选取痛风性关节炎湿热蕴结证患者90例,采用数字表法随机分为六黄散外敷组、四妙散口服组、六黄散外敷联合四妙散口服组,每组各30例。六黄散外敷组选取疼痛局部外敷六黄散,隔日1次,共治疗3次; 四妙散组给予四妙散口服,1剂/d,共6 d; 六黄散外敷联合四妙散口服组给予六黄散外敷与四妙散口服联合治疗。治疗前后检测各组患者血沉(ESR)、C 反应蛋白(CRP)、血尿酸(UA)水平,采用视觉模拟评分法(VAS)并根据《中医病证诊断疗效标准》拟定标准评定患者的临床疗效。结果:各组患者治疗后ESR、CRP、UA 水平及 VAS 评分与治疗前比较均明显降低,差异有统计学意义(P<0.05); 六黄散外敷与四妙散口服联合治疗组患者治疗后 ESR、CRP、UA 水平及 VAS 评分相比六黄散外敷组及四妙散口服组明显降低,差异有统计学意义(P<0.05)。六黄散外敷与四妙散口服联合治疗组患者临床疗效(93.3%)优于六黄散外敷组(60.0%)和四妙散口服组(63.3%),差异有统计学意义(P<0.05)。结论:六黄散外敷联合四妙散口服治疗急性痛风性关节炎湿热蕴结证疗效肯定,较两种疗法单独使用效果更佳。
Abstract:
Objective:To observe the clinical efficacy of Liuhuang powder combined with oral Simiao powder on the treatment of acute gouty arthritis syndrome of dampness-heat accumulation.Methods:90 patients with rheumatoid arthritis syndrome of dampness-heat accumulation were randomly divided into Liuhuang powder external application group,Simiao powder group and Liuhuang powder external application combined with oral Simiao powder group,with 30 cases in each group.Liuhuang powder external application group selected Liuhuang powder for external application on pain areas,once every other day for 3 times.Simiao powder group was given Simiao powder orally,1 dose a day for 6 d.Liuhuang powder combined with oral Simiao powder group was given Liuhuang powder combined with oral Simiao powder.Blood sedimentation rate(ESR),C-reactive protein(CRP)and blood uric acid(UA)levels were detected in each group before and after treatment,and visual analogue scale(VAS)was used to evaluate pain scores.Clinical efficacy of each group was compared after treatment.Results:After treatment,the levels of ESR,CRP,UA and VAS scores in each group were significantly lower than those before treatment(P<0.05).After treatment,the levels of ESR,CRP,UA and VAS scores of patients in Liuhuang powder external application combined with oral Simiao powder group were significantly decreased(P<0.05).The clinical efficacy of Liuhuang powder external application combined with oral Simiao powder group(93.3%)was better than that of Liuhuang powder external application group(60.0%)and Simiao powder group(63.3%)(P<0.05).Conclusion:External application of Liuhuang powder combined with oral administration of Simiao powder is effective on the treatment of acute gouty arthritis with dampness and heat accumulation syndrome,which is better than the two therapies application alone.
