摘要:
目的:探究血清基质细胞衍生因子-1(SDF-1)、降钙素原(PCT)水平对胫腓骨开放性骨折术后早期感染的诊断价值。方法:选取2019年6月至2020年11月在本院行钢板内固定手术治疗的142例胫腓骨开放性骨折患者,统计术后1个月四胫腓骨开放性骨折患者术后感染情况,并依据是否发生术后感染分为感染组和未感染组。对比感染组和未感染组临床资料,Logistic多因素回归分析影响胫腓骨开放性骨折患者发生术后感染的因素,制作受试者工作特征曲线(ROC),分析血清SDF-1、PCT及两者联合对胫腓骨开放性骨折术后感染的诊断效能。结果:胫腓骨开放性骨折患者术后1个月,术后感染率为25.35%(36/142)。感染组合并糖尿病占比、Gustily分型 Ⅲ型占比、手术时间、植皮占比、植骨占比、血清SDF-1及PCT水平均高于未感染组,差异有统计学意义(P<0.05)。Logistic回归多因素分析结果显示,合并糖尿病、Gustily分型 Ⅲ型、血清SDF-1、PCT均为胫腓骨开放性骨折患者发生术后感染的影响因素(OR=2.673、3.254、3.700、4.166)。ROC分析显示,血清SDF-1、PCT诊断胫腓骨开放性骨折术后早期感染的最佳截断点分别为185.27 pg/mL、2.16 ng/mL,两者联合的特异度为97.17%,高于血清SDF-1、PCT单独进行评估的特异度,且血清SDF-1、PCT两者联合诊断胫腓骨开放性骨折术后早期发生感染的曲线下面积(AUC)为0.874,高于血清SDF-1、PCT单独诊断的AUC,差异有统计学意义(P<0.05)。结论:血清SDF-1、PCT两者联合对胫腓骨开放性骨折术后早期感染的诊断效能较高,可为胫腓骨开放性骨折术后早期感染的诊断提供一定的依据。
Abstract:
To explore the value of serum stromal cell-derived factor-1(SDF-1)and procalcitonin(PCT)levels in the diagnosis of early postoperative infection of open tibia and fibula fractures.Methods:A total of 142 patients with open tibia and fibula fractures who underwent plate internal fixation in hospital from June 2019 to November 2020 were selected.The postoperative infection of patients with open fractures of the tibia and fibula 1 month after the operation was counted,and the postoperative infection cases were divided into an infected group and an uninfected group according to whether postoperative infection occurred.The clinical data of the infected group and the uninfected group were compared.The factors affecting postoperative infection in patients with open tibia and fibula fractures were analyzed by Logistic regression multivariate analysis.A receiver operating characteristic curve(ROC)was made to analyze the diagnostic efficacy of serum SDF-1,PCT and the combination of the two on postoperative infection of tibia and fibula open fractures.Results:One month after surgery,the postoperative infection rate of patients with open tibia and fibula fractures was 25.35%(36/142).The proportion of infection combined with diabetes,the proportion of gustilo type Ⅲ,the proportion of operation time,the proportion of skin grafts,the proportion of bone grafts,the serum SDF-1 and PCT levels were significantly higher than those of the uninfected group(P<0.05).Logistic regression multivariate analysis showed that diabetes mellitus,gustilo type Ⅲ,serum SDF-1,and PCT were influencing factors of postoperative infection in patients with open tibia and fibula fractures(OR=2.673,3.254,3.700,4.166).ROC analysis showed that the best cut-off points of serum SDF-1 and PCT for the diagnosis of early postoperative infection of tibia and fibula open fractures were 185.27 pg/mL and 2.16 ng/mL respectively.The specificity of the combination of the two was 97.17%,which was higher than that of serum SDF-1 and PCT assessed separately.The area under the curve(AUC)of early postoperative infection of tibia and fibula when combined with serum SDF-1 and PCT in the diagnosis of open tibia and fibula fractures was 0.874,which was significantly higher than that of serum SDF-1 and PCT AUC alone diagnosis(P<0.05).Conclusion:The combination of serum SDF-1 and PCT is highly effective in diagnosing early postoperative infection of open tibia and fibula fractures,and can provide a basis for the diagnosis of early postoperative infection of tibia and fibula open fractures.
