摘要:
目的:探讨尺骨变异对手法复位桡骨远端骨折预后的影响。 方法:回顾性分析2015年3月至2018年2月本科收治手法复位单侧桡骨远端骨折患者84例。依据尺骨变异(UV)变化大小分为高尺骨变异组(UV≥2.5 mm,n=31例)和低尺骨变异组(UV<2.5 mm,53例),比较两组患者临床指标(腕关节功能DASH评分、腕关节活动度、握力)、影像学指标(掌倾角、尺偏角)及并发症发生情况。结果:所有患者均获得临床随访并得到骨性愈合((8.86±3.21)周)。低尺骨变异组在随访6个月后VAS评分较高尺骨变异组出现了明显降低,差异有统计学意义(P=0.039,0.021)。末次随访时,低尺骨变异组DASH评分及握力分别为(13.82±9.47)分和(299.10±92.12)N,优于高尺骨变异组的(19.78±12.16)分和(242.06±87.20)N,差异有统计学意义(P<0.05)。在掌倾角、尺偏角方面,两组差异无统计学意义(P>0.05)。结论:尺骨变异变化大小对手法复位桡骨远端骨折预后有明显影响,在手法治疗过程中应积极考虑尺骨变异。
Abstract:
Objective:To investigate the effect of ulna variation on the function of patients with distal radial fracture by manual reduction.Methods:From March 2015 to February 2018,84 cases of unilateral distal radius fracture treated by manual reduction in our department were retrospectively analyzed.According to ulnar variance(UV)changes,all enrolled patients were divided into the high UV group(UV≥2.5 mm,31 cases)and the low UV group(UV<2.5 mm,53 cases).The clinical indexes(wrist joint function DASH score,wrist motion,grip strength),imaging indexes(palm inclination,ulnar deviation)and complications were compared.Results:All patients got fully follow-up and bone healing((8.86 +3.21)weeks).After 6 months,the VAS score of low-UV group was significantly lower than that of high-UV group(P=0.039,0.021). At latest follow-up, the DASH score and grip strength of the low UV group were(13.82±9.47)and(299.10±92.12)N respectively, which were better than those of the high UV group(19.78±12.16),(242.06±87.20)N(P<0.05). There was no significant difference in palm inclination and ulnar deviation(P>0.05). Conclusion: The ulna variance has a significant effect on the prognosis of distal radius fracture by manual reduction. The ulna variance should be considered actively.
