摘要:
目的:探究腰椎椎间盘退行性变与腰椎椎间孔狭窄间的关系,为临床经椎间孔侵入性治疗等提供解剖学依据。方法:在20套成人干燥腰椎骨上测量双侧L3~S1椎间孔的高度、椎间孔的最小矢状径及最大矢状径,并模拟椎间盘退行性变所致椎间隙狭窄时相应节段椎间孔大小的变化情况。结果:L3~4,L4~5和L5S1椎间孔平均高度分别为(19.27±1.78)mm,(17.45±1.88)mm,(14.01±1.38)mm; 椎间孔最小矢状径分别为(9.39±1.45)mm,(8.33±1.6)mm,(10.16±2.09)mm; 椎间孔最大矢状径分别为(11.14±1.28)mm,(10.95±0.85)mm,(11.13±1.17)mm.在移除椎间盘后,椎间孔高度、最小和最大矢状径均减小。各节段椎间孔高度在轻度和中度的椎间盘退行性改变时差异有统计学意义(P<0.05); 椎间孔最小矢状径在椎间盘高度正常时与椎间盘退行性改变时差异有统计学意义(P<0.05); L3~4和L4~5节段,椎间孔最大矢状径在椎间盘正常时与无椎间盘时差异有统计学意义(P<0.05)。结论:椎间盘退变所导致的椎间隙狭窄可造成椎间孔的高度明显减小,对椎间孔最小矢状径的影响最大; 而对椎间孔最大矢状径影响较小。在L5S1节段,椎间孔的最宽处与椎间隙的改变无关。
Abstract:
Objective:To investigate the association between the degeneration of intervertebral disc and the stenosis of lumbar intervertebral, providing anatomic basis for operations at intervertebral foramen and acupotomology. Methods: All 20 sets of dry adults' lumbar bone specimens were measured in foramen height, minimum foramen width, maximum foramen width. Stenosis of lumbar intervertebral foramen were stimulated. Results: The foramen height at L3~4,L4~5,L5S1 was(19.27±1.78)mm, L4~5 was(17.45±1.88)mm, L5S1 was(14.01±1.38)mm. The minimum foramen width at L3~4, L4~5, L5S1 was(9.39±1.45)mm,(8.33±1.6)mm,(10.16±2.09)mm; the maximum foramen width at L3~4, L4~5,L5S1 was(11.14±1.28)mm,(10.95±0.85)mm,(11.13±1.17)mm. The height of foramen decreased when the intervertebral disc was removed. At L3~4, L4~5, L5S1, the average decreased foramen height was 6.08 mm, 4.96 mm, 4.26 mm. When the disc was removed, the height of foramen, minimum foramen width, maximum foramen width was all decreased significantly. Conclusion:Stenosis of lumbar intervertebral foramen can lead to the decrease of the foramen height, minimum foramen width, maximum foramen width. At L5S1, no significant difference is observed between the maximum foramen and the different degrees of intervertebral disc herniation.
