摘要:
目的:探讨强直性脊柱炎患者骶髂关节炎X线分级与脊柱活动度的关系,为强直性脊柱炎的诊疗提供参考。方法:收集符合标准的129例临床诊断为AS的患者,借助骨盆平片检查确定骶髂关节炎分级,同时测定腰椎前屈、后伸、指地距、胸廓活动度,运用双变量Spearman相关和One way ANOVA方法进行统计分析。结果:1)腰椎前屈、后伸活动度和胸廓活动度与骶髂关节炎程度存在负相关关系(P<0.05),指地距与骶髂关节炎程度呈正相关关系(P<0.05); 2)Ⅲ级、Ⅳ级骶髂关节炎患者的指地距显著高于0～Ⅱ级的活动度,差异有统计学意义(P<0.05),而Ⅲ级和Ⅳ级骶髂关节炎病变的腰椎活动度、指地距并差异无统计学意义(P>0.05); 3)Ⅳ级骶髂关节炎的患者的胸廓活动度显著低于0～Ⅲ级的患者,差异有统计学意义(P<0.05)。结论:AS患者骶髂关节炎X线分级与脊柱活动度存在一定相关性,可作为AS病情诊疗的参考,且临床上治疗早期的AS患者就要加强腰部和胸廓的功能锻炼,以期做到“以动防残”。
Abstract:
Objective:To explore the correlation between spine movement and stage of sacroiliitis and to improve clinical treatment of ankylosing spondylitis(AS).Methods:129 patients with AS were enrolled.The stage of sacroiliitis was confirmed by pelvis X-ray and the spine movement was also measured,including lumbar spine front flexion,lumbar spine backward extension,finger-ground distance and thoracic activity.And statistical analysis was performed with one way ANOVA and bivariate correlations.Results:1)Spine movement,including lumbar spine front flexion,lumbar spine backward extension and thoracic activity,was significantly negative correlated to stage of sacroiliitis.Finger-ground distance was positive correlated to stage of sacroiliitis significantly.2)The lumbar spine front flexion and lumbar spine backward extension of AS patients with stage Ⅲ&Ⅳsacroiliitis was significantly poorer than patients with stage 0～Ⅱsacroiliitis.And finger-ground distance of AS patients with stage Ⅲ&Ⅳ sacroiliitis was significantly longer than that of patients with stage 0～Ⅱ sacroiliitis.No significant difference was visible in lumbar spine movement and finger-ground distance between patients with stage Ⅲ sacroiliitis and others with stageⅣsacroiliitis.3)The thoracic activity of patients with stage Ⅳsacroiliitis was significantly poorer than that of others with stage 0～Ⅲ sacroiliitis.Conclusion:The spine movement of AS patients was significantly correlated to the stage of sacroiliitis.The function exercise of lumbar spine and thorax in early stage is important to the treatment and recovery of AS.
