摘要:
目的:探讨血清软骨寡聚基质蛋白(COMP)和基质金属蛋白酶(MMP-13)在膝关节骨性关节炎(KOA)诊断中的应用价值。方法:69例KOA患者全面体格检查,记录患者的年龄、性别、体质量指数(BMI)、WOMAC 评分、HSS评分及X线片检查(按 Kellgre n 放射学诊断标准评级),采用酶联免疫吸附测定法( ELISA)方法测定KOA患者血清中的 COMP和MMP-13水平。选取同一时间段、同一地区来上海中医药大学附属曙光医院宝山分院体检中心的健康体检者作为对照组。结果:KOA 组患者血清COMP和MMP-13水平明显高于对照组,差异有统计学意义(P=0.001),KOA 组患者血清中COMP和MMP-13水平与K-L分级、WOMAC评分、HSS评分正相关,与BMI指数之间没有相关性(P=0.712)。结论:血清中 COMP和MMP-13 水平与 KOA关节功能和疼痛程度均呈正相关性,测定 KOA 患者血清中COMP和MMP-13水平对早期诊断、预防KOA及评定药物的临床效果具有重要意义。
Abstract:
Objective:To investigate the application value of cartilage oligomeric matrix protein(COMP)and matrix metalloproteinase-13(MMP-13)in the diagnosis of knee osteoarthritis.Methods:Total physical examination, age, sex, body mass index(BMI), WOMAC score, HSS score and X-ray examination of 69 patients with KOA were recorded. COMP and MMP-13 levels in serum of KOA patients were determined by enzyme-linked emmunsorbent assay(ELISA)method. Healthy people from the same period and the same area were selected as the control group.Results:The serum levels of COMP and MMP-13 in patients with KOA were significantly higher than those in the control group(P=0.001). There was a positive correlation between the serum levels of COMP, MMP-13 and K-L classification, WOMAC score and HSS score in the serum of patients with KOA, and there was no correlation between BMI and index(P=0.712).Conclusion:Serum levels of COMP and MMP-13 were positively correlated with the level of KOA joint function and pain. Detection of serum COMP and MMP-13 levels in patients with KOA is of great significance for early diagnosis, prevention of KOA and clinical evaluation of drugs.
