摘要:
目的:探讨关节腔内注射氨甲环酸对初次单侧人工全膝关节置换术后失血量的影响。方法:2014年6月至2015年6月选取膝骨性关节炎性初次单侧人工全膝关节置换术患者100例,采用随机数表方法随机分为两组,试验组50例,对照组50例。试验组于手术缝合关节囊时在关节腔注射氨甲环酸(0.25氨甲环酸溶入20 mL生理盐水),对照组常规缝合,术后第1,3,5天复查血常规并根据两组患者血红蛋白(Hb)及红细胞比容(HCT)计算失血量,比较术后输血例数、输血量。结果:两组患者术后隐性失血量、输血例数、输血量比较差异均有统计学意义(P<0.05)。结论:关节腔注射氨甲环酸在初次单侧全膝关节置换术后能明显降低患者术后失血量及输血率。
Abstract:
Objective:To explore the effect of tranexamic acid intra-articular injection on postoperative blood loss of primary unilateral total knee arthroplasty.Methods:From June 2014 to June 2015 100 patients with knee osteoarthritis underwent primary unilateral total knee arthroplasty were randomly divided into the experimental group and the control group according to a random number table,50 patients in each group.The experimental group was applied tranexamic acid intra-articular injection(0.25 tranexamic acid dissolved in 20 mL of normal saline)after joint capsule suture,and the control group underwent conventional suture.1,3,5 days after operation,the cases of postoperative blood transfusion,blood transfusion amount based on hemoglobin(Hb)and hematocrit(HCT)were calculate and compared.Results:The hidden blood loss postoperative,the cases of blood transfusion and amount of blood transfusion differences were statistically significant(P<0.05)in two groups.Conclusion:Tranexamic acid intra-articular injection after primary unilateral total knee replacement can significantly reduce postoperative blood loss and transfusion rates.
