摘要:
目的:探讨加味补中益气汤在高龄股骨粗隆间骨折患者围手术期的应用。方法:将62例高龄股骨粗隆间骨折患者随机分为对照组和治疗组各31例; 所有患者均行骨折闭合复位PFNA内固定治疗,围手术期内对照组采用常规对症处理,治疗组除常规治疗外同时配合服用加味补中益气汤,两组患者分别于入院1 d及术后1,7,14 d检测血清中IL-6,IL-10,TNF-α的含量。结果:两组患者在入院1 d血清中IL-6,IL-10,TNF-α含量差异无统计学意义(P>0.05); 术后血清中的IL-6,IL-10,TNF-α水平均呈现先升后降的趋势; 术后1 d两组患者的IL-6,TNF-α含量显著升高,对照组较治疗组升高明显(P<0.05),IL-10含量差异无统计学意义(P>0.05)。术后7,14 d血清中IL-6,IL-10,TNF-α含量下降,均低于术前水平,治疗组较对照组下降明显(P<0.01)。结论:使用加味补中益气汤可有效降低高龄股骨粗隆间骨折患者围手术期的炎症反应,保护组织和器官,有利于促进术后康复和减少术后并发症的发生。
Abstract:
Objective:To explore the application of Modified Buzhongyiqi Decoction on elderly patients with intertrochanteric fracture during the perioperative period.Methods:62 patients were randomly divided into the control group and the treatment group,31 cases in each group.All patients were received the PFNA internal fixation after closed reduction.The control group received routine treatment,and treatment group was additionally given Buzhongyiqi decoction.Contents of serum IL-6,IL-10 and TNF-α were detected in two groups on the first day of admission,and 1 d,7 d and 14 d after operation respectively.Results:On the first day of admission,there was no significant difference in the contents of of IL-6,IL-10 and TNF-α between two groups(P>0.05).The postoperative serum IL-6,IL-10 and TNF-α level were increased at first and then decreased.On the first day after operation,the content of IL-6 and TNF-in the two groups was significantly increased,and the control group was significantly higher than that in the treatment group(P<0.05),but there was no significant difference in the content of IL-10 between two groups(P>0.05).At 7 and 14 days after operation,the serum levels of IL-6,IL-10 and TNF-were decreased,all of which were lower than those before operation,and the treatment group decreased significantly compared with the control group(P<0.01).Conclusion:The application of Modified Buzhongyiqi Decoction could effectively reduce the inflammatory reaction,protect tissues and organs,improve postoperative rehabilitation and reduce the incidence of postoperative complications in elderly patients with intertrochanteric fracture during the perioperative period.
