摘要:
目的:比较经肌间隙入路伤椎置钉与传统跨伤椎置钉治疗单节段胸腰椎骨折的有效性及安全性。方法:回顾性分析比较本院2010年7月至2014年1月的76例A、B型单节段胸腰椎压缩性骨折患者(其中42例采用伤椎置钉内固定,34例采用传统的跨伤椎内固定,均未采用植骨融合)术前、术后1 d、术后3个月、6个月、12个月伤椎高度比及其Cobb角变化。结果:A型骨折患者在术前、术后1 d、术后3个月、术后6个月、术后12个月的伤椎高度比及Cobb角变化方面差异无统计学意义(P>0.05); B型骨折患者在术前、术后1 d、术后3个月的伤椎高度比及Cobb角变化方面差异无统计学意义(P>0.05),但在术后6个月及12个月的伤椎高度比及Cobb角变化方面差异有统计学意义(P<0.05)。结论:经伤椎置钉和跨伤椎置钉方式均能有效纠正A型胸腰椎骨折的脊柱后凸畸形、重建伤椎高度; 但对B型胸腰椎骨折来说经伤椎置钉术式在术后中长期可更好地维持伤椎复位高度和伤椎后凸畸形的矫正度数。
Abstract:
Objective:To compare the efficacy and safety of the treatment of single segment thoracolumbar fractures with vertebral pedicle screw and traditional cross injury vertebral screw by paraspinal approach. Methods:Comparative retrospective analyzed 76 patients, 42 patients treated with injured vertebra transpedicular fixation and 34 patients treated with traditional shorter segment fixation, in our hospital from 2010 to 2014. The injury vetebral height ratio and the Cobb angle changes were detected before surgery, the first day, 3 months, 6 months, and 12 months after surgery. Results:The injury vetebral height ratio and the Cobb angle changes were not significantly different in tape A fracture before surgery, the first day, 3 months, 6 months, and 12 months after surgery(P>0.05). The injury vetebral height ratio and the Cobb angle changes were not significantly different in tape B fracture before surgery, the first day and 3 months after surgery(P>0.05), but were significantly different 6 months and 12 months after surgery(P<0.05). Conclusion:The vertebral pedicle screw and traditional cross injury vertebral screw can both effectively correct the spine kyphosis of type A thoracolumbar fractures. But for the type B thoracolumbar fractures, the vertebral pedicle screw is better to maintain the height of the injured vertebra and the degree of correction of spine kyphosis for long term postoperative.
