摘要:
目的:观察关节腔冲洗联合注射玻璃酸钠治疗髋骨关节炎的有效性及其优势,为髋骨关节炎提供一种可行、有效的治疗方法。方法:通过对经关节腔冲洗联合玻璃酸钠治疗并获得半年随访的髋骨关节炎患者进行疗效观察,并以玻璃酸钠关节腔注射治疗的患者为对照组。根据纳入标准,纳入治疗组85例,对照组80例,以VAS评分、Harris评分、影像稳定率作为疗效评价指标,影像评价及数据分析委托第三方在盲态下完成,数据统计运用SPSS17.0软件进行统计学分析,两组间比较计数资料采用χ2检验,计量资料采用t检验,组内比较采用配对t检验。结果:治疗后半年,两组患者VAS评分均低于治疗前(P<0.001),Harris总分以及疼痛、能力、畸形、活动度均高于治疗前(P<0.001)。VAS评分治疗组低于对照组(P<0.001); Harris评分优良率治疗组高于对照组(P<0.05),Harris总分以及疼痛程度及能力得分,治疗组高于对照组(P<0.001),关节畸形及活动度得分两组间差异无统计学意义(P>0.05); 影像学稳定率治疗组与对照组之间无显著性差异(P>0.05)。结论:对于髋骨关节炎患者,无论采用关节腔注射玻璃酸钠,还是关节腔冲洗联合注射玻璃酸钠治疗,均有良好的疗效; 关节腔冲洗联合注射玻璃酸钠在缓解疼痛症状、改善关节功能方面效果更具优势。
Abstract:
Objective: To observe the effectiveness and superiority of intra-articular washing combined with sodium hyaluronate injection in treatment of hip osteoarthritis, providing a feasible and effective treatment for hip osteoarthritis. Methods:By observing the efficacy of hip osteoarthritis patients being treated with combined intra-articular washing and sodium hyaluronate injection who was being followed up for six months. According to the inclusion criteria, there were 85 cases in the treatment group(intra-articular washing combined with sodium hyaluronate injection),80 cases in the control group(intra-articular injection of sodium hyaluronate).VAS score, Harris score and iconography stabilization rate was used as the efficacy evaluation index. Image evaluation and data analysis was completed by a third party blinded. SPSS17.0 software was used for statistical analysis between the two groups, the count data comparison with χ2 test, the measurement data with t-test and paired t-test was used for inner group comparison.Results: The VAS scores six months after treatment were lower than that before treatment in both groups(P<0.001). Harris scores,pain, ability, deformity and activity were higher than that before treatment(P<0.001). VAS scores in treatment group was lower than that in the control group(P<0.001).Harris scores excellent rate in treatment group was higher thah that in control group(P<0.05).Harris scores, pain intensity and ability score were higher those in the treatment group(P<0.001).Joint deformity and activity score has no difference between two groups(P>0.05).There was no significant difference of iconography stabilization rate between two groups(P>0.05). Conclusion: For hip osteoarthritis patients, no matter intra-articular injection of sodium hyaluronate or intra-articular washing combined with sodium hyaluronate injection can both have a good efficacy. Intra-articular washing combined with sodium hyaluronate injection is more superior in relieving pain and improving joint function.
