摘要:
目的:探讨颈后路选择性减压侧块螺钉内固定治疗无骨折脱位颈脊髓损伤的临床应用效果。方法:共收治21例无骨折脱位颈脊髓损伤患者,行后路选择性减压侧块螺钉内固定手术治疗,采用JOA评分评价脊髓功能的变化及影像学手段评价内固定与植骨融合情况。结果:20例患者得到有效随访,85%患者神经功能得到不同程度改善。随访JOA评分较术前显著提高(P<0.05),总有效率80%.术后随访颈椎X线检查显示所有患者内固定位置良好,术后半年复查颈椎CT平扫+三维重建显示所有患者的后路植骨均融合。结论:后路选择性减压并侧块螺钉内固定技术是治疗无骨折脱位颈脊髓损伤的有效方法,在达到有效减压固定的同时,相对减少了颈椎后方组织的破坏。
Abstract:
Objective: To investigate the effect of selective posterior decompression and lateral mass screw fixation for cervical spinal cord injury without fracture and dislocation. Methods: A total of 21 cases of patients with cervical spinal cord injury without fracture and dislocation were treated with selective posterior decompression and lateral mass screw fixation operation. They were all followed-up. The changed function of spinal cord was evaluated by JOA score, and the effect of the internal fixation and bone grafting fusion was evaluated by iconography method. Results: Twenty cases were got effective followed-up. The neurological function was improved in 85% patients. The JOA score was significantly increased after treatment(P<0.05), and the total effective rate was 80%. The X-ray of cervical showed that the position of internal fixation of all patients were good. The CT scan and three-dimensional reconstruction showed that the bone grafting of all patients were confertus half a year after surgery. Conclusion: The selective posterior decompression and lateral mass screw fixation is effective method for treatment of cervical spinal cord injury without fracture and dislocation. It can achieve effective decompression and fixation, meanwhile relatively reduce the destruction of posterior cervical tissue.
