摘要:
目的:观察术中连续浸润氨甲环酸对减少全髋关节置换术后失血的疗效及安全性。方法:采用前瞻性的随机对照研究方法,将我科2012年1月-2013年8月收治的214例拟行单侧全髋关节置换手术的患者按随机数字表法进行随机分组,分为观察组和对照组,每组107例。观察组术中分别用氨甲环酸对髋臼、股骨髓腔及整个关节囊进行浸润; 对照组术中不使用氨甲环酸。两组患者术后对比引流量、血红蛋白、总失血量、总输血量、凝血酶原时间、活化部分凝血活酶时间、D-二聚体、以及术后下肢深静脉血栓(DVT)的发生率。结果:术前两组患者一般情况无统计学差异(P>0.05),观察组术后引流量、总失血量、总输血量明显低于对照组(P<0.05),血红蛋白量均高于对照组(P<0.05),两组患者术后凝血酶原时间、活化部分凝血活酶时间、D-二聚体、以及术后DVT的发生率比较无统计学差异(P>0.05)。结论:采用术中连续浸润氨甲环酸可明显的减少全髋关节置换术后出血,减少患者输血量,且并无增加患者DVT的风险。
Abstract:
Objective: To observe the efficacy and safety of continuous intraoperative infiltration of tranexamic acid in reducing postoperative blood loss of totaL hip replacement. Methods: Totally 214 patients from Jan 2012 to Aug 2013 who received unilateral total hip arthroplasty were randomly divided into two groups with prospective and randomized study, 107 cases in each group. The experimental group was infiltrated with tranexamic acid on the acetabulum, femoral canal and the entire articular capsule, while the control group without. The postoperative drainage volume, hemoglobin, total blood loss, the total blood transfusion, prothrombin time, activated partial thromboplastin time, D-dimmer and the incidence of postoperative DVT were compared between the two groups. Results: The general health condition of the two groups before treatment had no significant difference(P>0.05).The postoperative drainage, total blood loss and total blood transfusion in the experimental group were significantly lower than those in the control group(P<0.05),while the hemoglobin was higher than that in the control group(P<0.05).The postoperative prothrombin time, activated partial thromboplastin time, D-dimmer and the incidence of postoperative DVT had no significant difference between two groups(P>0.05).Conclusion:Continuous intraoperative infiltration of tranexamic acid can obvious reduce the postoperative blood loss and the amount of blood transfusion in total hip replacement without risk of DVT.
