摘要:
总结姜宏教授治疗急性非特异性滑膜炎的临床经验,姜宏教授认为本病核心病机为湿热痹阻,与外伤、劳损、外邪等因素导致筋脉受阻、水液输布失常密切相关。强调急性期以治标为先,立清热利湿、通络止痛为治疗原则。其特色在于内服外治相结合,内以廓清饮联合四妙丸为基础方,廓清饮功擅行气开壅、利水消肿,以调畅三焦气机而促进水湿代谢,四妙丸专于清利湿热,二者协同,从不同角度清除湿热之邪。外治则运用苏州吴门医派葛氏伤科自制金黄膏,敷于患处后以绷带固定,发挥清热解毒、消肿止痛之功,其固定作用亦有助于关节制动,“内外合治”加强临床疗效。
Abstract:
This article summarizes Professor Jiang Hong's clinical experience in treating acute non-specific synovitis.Professor Jiang Hong believes that the core pathogenesis of this condition is damp-heat obstruction.It is closely related to factors such as trauma and strain,which lead to the obstruction of Qi and blood circulation.He emphasized that during the acute phase,the focus should be on addressing the symptoms-specifically,clearing heat,resolving dampness,and reducing swelling.Treatment features the combination of internal and external therapies,with oral administration of Kuoqing decoction combined with Simiao pills.Kuoqing decoction is effective for regulating Qi and promoting diuresis.Simiao pills are specialized for resolving damp-heat in the triple burner.The two formulas work synergistically to eliminate damp-heat pathogens from different angles.For external treatment,the self-made Jinhuang ointment is applied to the joint and fixed with a bandage.It has the effects of clearing heat,reducing swelling and relieving pain.Its stabilizing effect also helps to immobilize the joints and prevent secondary injuries.
