摘要:
目的:探讨拇趾外翻患者采用三维打印截骨导板辅助Chevron截骨术的临床效果,寻找更精确的治疗方式。方法:研究40例中重度拇趾外翻患者,随机分为两组,观察组采用三维打印截骨导板辅助Chevron截骨术治疗,对照组采用传统器械进行Chevron截骨术治疗,比较分析两组患者手术切口愈合情况、手术时间、术后负重位第一跖骨间夹角(IMA)的改善情况及术后并发症。结果:两组患者基本资料、术前负重位第一跖骨间夹角和美国足踝外科协会(AOFAS)评分差异无统计学意义(P>0.05); 观察组术后第一跖骨间夹角为5.98°±1.23°,对照组为10.61°±2.69°,两组差异有统计学意义(P<0.05); 观察组AOFAS评分优良率为100%,对照组为65%,两组差异有统计学意义(P<0.05); 观察组手术时间为(0.88±0.17)h,对照组手术时间为(1.51±0.51)h,两组差异有统计学意义(P<0.05)。结论:利用数字化医学联合三维打印技术,于术前模拟制定矫形方案,术中制作截骨导板指导精准定位并辅助截骨矫形,为拇趾外翻患者提供个体化、精准化的矫形治疗方案,值得借鉴和参考。
Abstract:
Objective:To explore the clinical effect of using a three-dimensional printing osteotomy guide plate for Chevron osteotomy in patients with hallux valgus,and to find a more accurate treatment method.Methods:40 patients with moderate to severe hallux valgus were randomly divided into two groups.The observation group was treated with Chevron osteotomy assisted by a three-dimensional printing osteotomy guide plate,and the control group was treated with Chevron osteotomy using traditional instruments.The wound healing of the surgical incision,operation duration,improvement of the intermetatarsal angle(IMA)in the weight-bearing position after surgery,and postoperative complications of the two groups were compared and analyzed.Results:There was no statistical significance in the basic data,pre-operative weight-bearing IMA,and AOFAS scores between the two groups.The postoperative IMA of the observation group was 5.98°±1.23°,and that of the control group was 10.61°±2.69°,with a statistically significant difference between the two groups(P<0.05).The excellent and good rate of AOFAS scores in the observation group was 100%,while that in the control group was 65%,with a statistically significant difference between the two groups(P<0.05).The operation time of the observation group was(0.88±0.17)h,and that of the control group was(1.51±0.51)h,with a statistically significant difference between the two groups(P<0.05).Conclusion:Using digital medicine combined with three-dimensional printing technology,an orthopedic plan is simulated and formulated before the operation.During the operation,an osteotomy guide plate is fabricated to guide precise positioning and assist in osteotomy and orthopedic procedures.This provides individualized and precise orthopedic treatment plans for patients with hallux valgus,which is worthy of reference.
