摘要:
目的:探讨关节镜下重建前交叉韧带(ACL)中保留残端纤维对膝关节功能及本体感觉影响。方法:选择2009年4月-2013年11月70例关节镜下ACL重建术患者,按照术式分为观察组与对照组,观察组35例患者重建术中鞘内保留残端重建,对照组35例患者重建中未保留残端,对比两组患者膝关节功能IKDC、Lysholm评分,膝关节稳定性轴移试验、Lachman试验、KT-2 000试验结果,以及术后12个月双膝关节被动角度再生试验结果。结果:观察组术后6个月、12个月IKDC、Lysholm评分分别为(74.94±4.44)、(92.53±6.57);(82.17±6.47)、(93.46±6.68),均优于对照组,2组比较差异有统计学意义(t=4.227,P=0.015; t=3.994,P=0.024; t=4.057,P=0.016; t=3.247,P=0.034); 两组患者术后轴移、Lachman试验结果与术前改善明显,对比差异有统计学意义(P<0.05),两组间比较差异均无统计学意义(P>0.05); 观察组术后6个月、12个月KT-2 000评分为(0.84±0.17)、(1.03±0.21),优于对照组,对比差异有统计学意义(t=3.107,P=0.038; t=4.278,P=0.013); 对照组30°、60°、90°患侧健侧对比差异有统计学意义(P<0.05),观察组30°、60°、90°患侧健侧对比差异均无统计学意义(P>0.05)。结论:关节镜下重建前交叉韧带中保留残端纤维对膝关节功能及本体感觉恢复有利。
Abstract:
Objective: To investigate the effect of arthroscopic ACL reconstruction surgery with reserved stump fiber for knee function. Methods: All 70 cases of arthroscopic ACL reconstruction patients were divided into observation group and control group(35 cases in each group)from April 2009 to November 2013. The patients of observation group were implemented with intrathecal reserved stump reconstruction, while the patients of control group were implemented without stump. The knee function of IKDC, Lysholm score, knee stability pivot shift test, Lachman test, KT-2000 test results, and double knee passive angle regeneration results of 12 months between two groups were compared. Results: The IKDC and Lysholm score of observation group were(74.94±4.44),(92.53±6.57)and(82.17±6.47),(93.46±6.68)respectively at 6th months and 12th months, which were better than the control group(t=4.227, P=0.015; t=3.994, P=0.024; t=4.057, P=0.016; t=3.247, P=0.034). The postoperative pivot shift and the Lachman test results were improved significantly after surgery(P<0.05). There was no significant difference between the two groups(P>0.05). The KT-2000 score was(0.84±0.17)and(1.03±0.21)respectively in observation group at 6th months and 12th months, which were better than the control group(t=3.107, P=0.038; t=4.278, P=0.013). There was significant difference between ipsilateral and contralateral for 30 °, 60 ° and 90 ° in control group(P<0.05), and there was no significant difference in the observation group(P>0.05). Conclusion: Retaining stump in arthroscopic anterior cruciate ligament reconstruction is helpful to knee joint function and proprioception recovery.
