摘要:
目的:探讨芍药甘草汤治疗大鼠脊髓损伤后肌痉挛的药物配比,为临床使用该方提供依据。方法:70只大鼠分为假手术组、模型组、巴氯芬组、芍甘(1,2,3,4)组,每组各10只,假手术组以T9为中心行椎板切开后不对脊髓做任何处理,其余6组采用T9脊髓半横断法制作脊髓损伤模型。所有大鼠造模成功后进行随机分组,假手术组与模型组给予0.9%的氯化钠溶液灌胃,巴氯芬组以1.35 mg/kg剂量的巴氯芬溶液灌胃,芍甘(1,2,3,4)组分别以芍药、甘草比为1:1,2:1,3:1,6:1的芍药甘草汤浓缩液(浓度为0.4 g/mL)3.2 g/kg剂量灌胃,灌胃1次/d,连续3周。采用改良Ashworth评分评价用药前后肌张力变化情况,酶联免疫法(ELISA)测定脊髓组织内谷氨酸(GLU)、γ-氨基丁酸(GABA)及5-羟色胺(5-HT)的表达水平。结果:除假手术组与模型组外,其余各组肌张力较用药前均有显著降低,差异有统计学意义(P<0.05),其中芍甘3组降低程度最为明显,但与其他组相比差异无统计学意义(P>0.05)。与假手术组相比,模型组脊髓组织内谷氨酸水平升高,γ-氨基丁酸、5-羟色胺水平降低,差异有统计学意义(P<0.05)。与模型组比较,巴氯芬组与芍甘1,2,3,4组脊髓组织内谷氨酸水平有不同程度降低,但差异无统计学意义(P>0.05); 而γ-氨基丁酸与5-羟色胺水平有不同程度升高,差异有统计学意义(P<0.05),其中芍甘3组的γ-氨基丁酸与5-羟色胺水平升高幅度最为明显,并与芍甘1组相比差异有统计学意义(P<0.05)。结论:芍药甘草汤能通过提高脊髓损伤后肌痉挛大鼠脊髓组织内γ-氨基丁酸与5-羟色胺含量,从而缓解肌痉挛状态; 当芍药甘草汤中芍药、甘草比例为3:1时,γ-氨基丁酸与5-羟色胺含量变化最为明显,其舒缓痉挛效果最佳。
Abstract:
Objective:To investigate the drug ratio of shaoyaogancao decoction in treating muscle spasm after spinal cord injury(SCI)in rats,and to provide evidence for clinical use of Shaoyao Gancao decoction.Methods:70 rats were divided into sham operation group,model group,baclofen group,shaogan 1,2,3 and 4 groups,with 10 rats in each group.In the sham operation group,the spinal cord was not treated after laminotomy centered on T9,and the spinal cord injury model was made by T9 spinal cord hemisection in the other six groups.All rats were randomly divided into sham operation group and model group.0.9% sodium chloride solution was intragastrically administered to sham operation group; baclofen group was intragastrically administered with baclofen solution at a dose of 1.35 mg/kg,and Shaogan 1,2,3,and 4 groups were intragastrically administered with peony and licorice concentrate(concentration of 0.4 g/mL)at a dose of 3.2 g/kg at a ratio of 1:1,2:1,3:1,and 6:1,respectively,once a day for 3 weeks.The modified Ashworth score was used to evaluate the changes of muscle tone before and after treatment,and the expression levels of GLU,GABA and 5-HT in spinal cord tissues were determined by euzyme-linked immunosorbent assay(ELISA).Results:Except for sham operation group and model group,the muscle tension of other groups was significantly lower than that before treatment(P<0.05),and the degree of decrease was most obvious in Shaogan 3 group,but there was no significant difference compared with other groups(P>0.05).Compared with the sham group,GLU levels increased and GABA and 5-HT levels decreased in the spinal cord tissue of the model group,and the difference was statistically significant(P<0.05).Compared with the model group,the GLU levels in the spinal cord tissue of the baclofen group and the Shaogan 1,2,3,and 4 groups were decreased to different extents,but the difference was not statistically significant(P>0.05); while the GABA and 5-HT levels were increased to different extents,and the difference was statistically significant(P<0.05),in which the GABA and 5-HT levels of the Shaogan 3 group increased most significantly,and the difference was statistically significant compared with the Shaogan 1 group(P<0.05).Conclusion:Shaoyao Gancao decoction can relieve the spasticity by increasing the contents of GABA and 5-HT in the spinal cord tissue of rats with spasm after spinal cord injury,and the changes of GABA and 5-HT contents are most obvious when the ratio of paeonia lactiflora and glycyrrhiza uralensis is 3:1 in Shaoyao Gancao decoction,and its spasmolytic and cramps effect is the best.
