摘要:
目的:探讨三维(3D)打印截骨导板技术在膝内翻畸形全膝关节置换术中提高疗效的可行性。方法:选取2018年1月至2020年12月收治的126例膝内翻畸形患者,随机分为对照组和3D打印导板组。对照组使用传统人工全膝关节置换术(TKA)治疗,3D打印导板组使用3D打印截骨导板技术联合TKA治疗。记录两组患者的年龄、性别、手术时间、术中失血量、术后24 h引流量、术后3个月股骨与胫骨机械轴夹角(aMFTA)、膝关节KSS、WOMAC评分。结果:纳入研究共126例患者,其中男37例,女89例; 年龄为65~70岁。两组患者的年龄、性别、部位差异无统计学意义(P>0.05); 3D打印导板组手术时间为(62.57±3.87)min,优于对照组(86.74±3.87)min,差异有统计学意义(t=15.64,P<0.001); 3D打印导板组术中失血量为(140.67±11.36)mL优于对照组(186.57±12.37)mL,差异有统计学意义(t=23.41,P<0.001); 3D打印导板组术后24 h引流量(215.47±11.28)mL,优于对照组(289.57±11.38)mL,差异有统计学意义(t=28.54,P<0.001)。3D打印导板组术后3个月aMFTA夹角为(2.45±0.21)°,优于对照组(1.96±0.14)°,差异有统计学意义(t=6.28,P<0.001)。3D打印导板组术后3个月KSS临床评分为(87.56±1.58)分,与对照组(87.32±1.65)分相比,差异无统计学意义(t=1.38,P=0.66)。3D打印导板组术后3个月KSS功能评分为(87.61±1.62)分,优于对照组(87.58±1.57)分,差异有统计学意义(t=3.65,P=0.03)。3D打印导板组术后3个月WOMAC评分为(11.56±1.28)分,与对照组(12.13±1.34)分相比,差异无统计学意义(t=0.12,P=0.89)。结论:三维打印截骨导板技术在膝内翻畸形全膝关节置换术中较传统全膝关节置换术具有时间短、失血量少、提高手术截骨精准度和术后功能恢复快的优点,效果显著,值得临床推广应用。
Abstract:
Objective:To explore the feasibility of three-dimensional(3D)printing osteotomy guide in total knee arthroplasty for knee varus deformity.Methods:126 patients with genu varus from January 2018 to December 2020 were randomly divided into control group and 3D printing guide plate group.The control group was treated with traditional total knee arthroplasty(TKA),and the 3D printing guide plate group was treated with 3D printing osteotomy guide plate technology combined with TKA.The age,gender,operation time,intraoperative blood loss,24 h drainage flow,the angle between femoral and tibial mechanical axis(aMFTA),knee KSS and WOMAC scores of the two groups were recorded.Results:A total of 126 patients were included in this study,including 37 males and 89 females.The age ranged from 65 to 70 years old.Patients were randomly divided into control group and 3D printing guide plate group.There was no significant difference in age,gender and location between the two groups(P>0.05).The operation time of 3D printing guide plate group((62.57±3.87)min)was better than that of the control group((86.74 ±3.87)min),and the difference was statistically significant(t=15.64,P<0.001).The intraoperative blood loss in the 3D printing guide plate group((140.67±11.36)mL)was better than that in the control group((186.57±12.37)mL),and the difference was statistically significant(t=23.41,P<0.001).The 24 h drainage volume((215.47±11.28)mL)in the 3D printing guide plate group was better than that in the control group((289.57±11.38)mL),and the difference was statistically significant(t=28.54,P<0.001).The included angle of aMFTA in the 3D printing guide plate group((2.45±0.21)°)was better than that in the control group((1.96±0.14)°),and the difference was statistically significant(t=6.28,P<0.001).There was no significant difference in KSS clinical score((87.56±1.58)points)3 months after operation between 3D printing guide plate group and control group((87.32±1.65)points)(t=1.38,P=0.66).The KSS function scores of the 3D printing guide plate group(87.61±1.62)was better than that of the control group(87.58±1.57),and the difference was statistically significant(t=3.65,P=0.03).There was no significant difference in WOMAC scores(11.56±1.28)between 3D printing guide plate group and control group(12.13±1.34)(t=0.12,P=0.89).Conclusion:Compared with the traditional total knee arthroplasty,three-dimensional printing osteotomy guide technology has the advantages of short time,less blood loss,effectively improve the accuracy of osteotomy and rapid postoperative functional recovery.It has remarkable efficacy and is worthy of clinical application.
