摘要:
目的:对纤维血凝块填塞联合关节镜下FasT-Fix修复半月板撕裂的愈合相关因素进行探究。方法:选取2016年5月至2017年5月本院确诊的139例半月板损伤患者作为研究对象,行纤维血凝块填塞联合关节镜下FasT-Fix术修复半月板。记录患者年龄、性别、病程、BMI、撕裂类型、外伤史、术后制动时间等基本资料; 行膝关节MRI检查观察缝合部位恢复情况; 根据Barrett标准对患者术后6个月门诊随访检查。术前和术后末次观察患者症状,对患者进行IKDC评分、Lysholm 评分和JOY评分。结果:对影响半月板愈合的相关因素进行分析,结果显示年龄、性别、撕裂类型、术后制动时间等均不是影响半月板愈合的相关因素。BMI>25 kg/m2、病程>6周、外伤史不明确的患者半月板愈合率较低,差异均有统计学意义(P<0.05),三者为影响半月板愈合的相关因素。多因素Logistic回归分析结果显示,BMI、病程、外伤史为影响半月板愈合的相关因素; 术后膝关节活动尚未出现交锁症状,愈合较好; 术后IKDC评分、Lysholm评分、JOY评分显著高于术前,组间差异有统计学意义(P<0.05)。结论:纤维血凝块填塞联合关节镜下FasT-Fix修复半月板撕裂疗效佳,其中BMI、病程、外伤史为影响半月板愈合的相关因素,在患者围手术期应予以关注,避免愈合延迟。
