摘要:
目的:探讨针刀松解结筋病灶点调控Wnt/β-catenin信号通路对软骨退化的影响。方法:选择18只2月龄雄性新西兰兔标记编号,按照随机数字法分为空白组、模型组和针刀组,每组6只。空白组正常饲养,无制动操作; 模型组和针刀组采用Videman法进行兔膝关节炎造模,时间为6周。造模成功1周后,针刀组接受针刀干预,选取兔鹤顶次、髌外下、髌内下三个结筋病灶点,1次/周,共干预2周。治疗完成1周后,采用Lequesne MG膝关节评分评估膝关节炎严重程度; 苏木精-伊红染色观察滑膜组织和股直肌结构; 番红-固绿染色观察软骨损伤情况; 测量各组凝血和血流变纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、血浆凝血酶原时间(PT); 采用Western Blot和实时荧光定量PCR验证Wnt-3α、MMP-13、β-catenin和C-MYC的蛋白和mRNA表达水平。ELISA检测兔血清IL-1β、TNF-α、P-选择素、PGE2、cAMP、cGMP水平。结果:与模型组相比,针刀组Lequesne MG评分明显降低,差异有统计学意义(P<0.05)。组织学染色显示,针刀组纤维增生减少,软骨细胞分布更均匀。凝血检测显示针刀组凝血酶原时间延长,纤维蛋白原水平降低,差异有统计学意义(P<0.05)。针刀组软骨组织中Wnt-3α、MMP-13、β-catenin和C-MYC蛋白及mRNA的表达水平显著降低,差异有统计学意义(P<0.05)。ELISA分析显示,针刀组血清中的炎症因子(IL-1β、TNF-α、P-选择素、PGE2)水平显著降低,cAMP/cGMP比值升高,差异有统计学意义(P<0.05)。结论:针刀松解兔鹤顶次、髌外下、髌内下三个膝结筋病灶点,在改善膝关节功能、保护软骨组织、抑制炎症、调节兔软骨稳态和凝血功能反应方面具有显著效果,其机制可能与Wnt/β-catenin信号通路有关。
Abstract:
Objective:To explore the effects and mechanisms of acupotomy at these three Jingjin Lesion sites on cartilage degeneration in knee osteoarthritis.Methods:Eighteen 2-month-old male New Zealand rabbits were selected and marked with numbers. They were divided into three groups—a blank group,a model group,and an acupotomy group—with six rabbits in each group,using the random number method. The blank group was raised normally without immobilization. The model group and the acupotomy group were modeled with knee osteoarthritis using the Videman method for 6 weeks. One week after successful modeling,the acupotomy group received acupotomy intervention at three specific acupoints: Hedingci,the lateral superior patellar point,and the medial superior patellar point. The intervention was performed once a week for a total of 2 weeks. One week after the completion of the treatment,the severity of knee osteoarthritis was assessed using the Lequesne MG knee score. Hematoxylin-eosin staining was used to observe the synovial tissue and rectus femoris structure. Safranin O-fast green staining was used to observe the cartilage damage. The levels of coagulation and hemorheological parameters,including FIB,APTT,TT,and PT,were measured in each group. Western Blot and real-time quantitative PCR were used to verify the protein and mRNA expression levels of Wnt-3α,MMP-13,β-catenin,and C-MYC. ELISA for rabbit serum IL-1β,TNF-α,P-selectin,PGE2,cAMP,cGMP levels.Results:Compared to the model group,the acupotomy group exhibited significantly lower Lequesne MG scores(P<0.05). Histological staining revealed decreased fibrosis and a more uniform chondrocyte distribution in the acupotomy group. Coagulation tests showed prolonged PT and reduced FIB levels in the acupotomy group. Western Blot and qRT-PCR results demonstrated that acupotomy significantly downregulated the expression of Wnt-3α,MMP-13,β-catenin,and C-MYC at both protein and mRNA levels(P<0.05). ELISA analysis indicated lower levels of inflammatory factors(IL-1β,TNF-α,P-selectin,PGE2)and an elevated cAMP/cGMP ratio in the acupotomy group(P<0.05).Conclusion:Acupotomy therapy at the three Jingjin lesion sites demonstrated significant therapeutic effects in improving knee joint function,protecting cartilage tissue and its extracellular matrix,modulating coagulation function,and suppressing inflammatory responses. These effects are likely mediated through regulation of the Wnt/β-catenin signaling pathway.
