摘要:
目的:分析中老年人腓肠肌和跖肌肌肉的脂肪浸润与膝骨关节炎进展的相关性。方法:回顾性分析2023年1月至2025年2月住院并诊断为原发性单/双侧膝关节病患者80例,采用相关性分析及多因素线性回归分析小腿后群肌肉脂肪浸润与膝关节参数变化、骨关节炎进展程度之间的关联性。结果:膝骨关节炎患者的膝关节内侧间隙与腓肠肌脂肪浸润率(r=-0.027,P<0.001)、跖肌脂肪面积(r=-0.130,P<0.001)、跖肌脂肪浸润率(r=-0.043,P<0.001)呈负相关性; 膝关节外侧间隙与腓肠肌脂肪浸润率呈负相关性(r=-0.016,P=0.001)。在线性回归分析中,腓肠肌脂肪面积增大、脂肪浸润率升高,对膝关节内外侧间隙有负向影响关系,对股骨远端外侧角有正向影响关系。结论:中老年人小腿腓肠肌脂肪浸润率与膝关节内外侧间隙存在关联性; 同时,腓肠肌肌肉质量可能是评估中老年人膝关节退变程度、膝骨关节炎发生发展的重要指标之一,也可能是临床早中期防治膝骨关节炎的关键靶点之一。
Abstract:
Objective:To investigate the correlation between fat infiltration of the gastrocnemius and metatarsal muscles in middle-aged and elderly people and the progression of knee osteoarthritis.Methods:A retrospective analysis was conducted on a total of 80 patients who were hospitalized and diagnosed with primary unilateral or bilateral knee joints between January 2023 and February 2025.Correlation analysis and multivariate linear regression analysis were used to analyze the association between muscle fat infiltration in the posterior compartment of the lower leg and both knee joint parameter changes and osteoarthritis progression severity.Results:The medial knee joint space of patients with knee osteoarthritis was negatively correlated with the fat infiltration rate of the gastrocnemius muscle(r=-0.027,P<0.001),the fat area of the metatarsal muscle(r=-0.130,P<0.001),and the fat infiltration rate of the metatarsal muscle(r=-0.043,P<0.001).The lateral space of the knee joint was negatively correlated with the fat infiltration rate of the gastrocnemius muscle(r=-0.016,P=0.001).In the linear regression analysis,an increase in the fat area and fat infiltration rate of the gastrocnemius muscle is associated with a reduction in the medial and lateral joint spaces of the knee and an increase in the lateral angle of the distal femur.Conclusion:There is a correlation between the fat infiltration rate of the gastrocnemius muscle in the lower leg of middle-aged and elderly people and the medial and lateral spaces of the knee joint.Meanwhile,the mass of the gastrocnemius muscle may be one of the important indicators for evaluating the degree of knee joint degeneration and the occurrence and development of KOA in middle-aged and elderly people; it may also become one of the key targets for the prevention and treatment of KOA in the early and middle stages of clinical practice.
