摘要:
目的:观察手法复位小夹板固定联合“蛇难爬”消肿散改良方外敷对老年人闭合性C1型Colles骨折的临床疗效及其对骨代谢指标的影响。方法:选取2022年1月至2023年9月收治的老年人C1型Colles骨折患者50例作为研究对象,依据治疗方法的不同分为观察组(25例)和对照组(25例)。观察组患者采用手法复位小夹板固定联合“蛇难爬”消肿散改良方外敷方案,对照组患者采用手法复位小夹板固定并用普通无菌纱布外敷方案。比较两组患者临床疗效及治疗后疼痛、肿胀程度、关节解剖结构关系、骨折愈合情况、腕关节功能、骨质代谢指标。结果:观察组患者在腕部肿胀消退程度、视觉模拟量表(VAS)评分、断端骨痂生长时间、腕关节功能方面均优于对照组,差异有统计学意义(P<0.05),而在腕关节解剖结构关系方面两组患者差异无统计学意义(P>0.05),但明显好于治疗前,差异有统计学意义(P<0.01),直至治疗后第12周差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后骨钙素水平和血清总Ⅰ型胶原氨基端升高,且观察组变化和对照组相比更显著,差异有统计学意义(P<0.05)。结论:手法复位小夹板固定能恢复并维持腕关节的正常解剖结构关系,联合“蛇难爬”消肿散改良方外敷可显著减轻骨折处疼痛、肿胀,改善骨代谢,促进老年患者骨折愈合。
Abstract:
Objective:To observe the clinical efficacy of manual reduction and small splint fixation combined with “snake difficult to climb” anti-swelling powder modified prescription external application on the elderly patients closed C1 Colles fracture and its effect on bone metabolism indexes.Methods:50 elderly patients with C1 Colles fracture from January 2022 to September 2023 were selected as the study objects,and were divided into observation group(25 cases)and control group(25 cases)according to different treatment methods.The observation group was treated with manual repositioning small splint fixation combined with “snake difficult to climb” anti-swelling powder modified prescription external application scheme,while the control group was treated with manual repositioning small splint fixation and ordinary sterile gauze external application scheme.The clinical efficacy and the relationship between pain,swelling,anatomical structure,fracture healing,wrist function evaluation and bone metabolism indexes were compared between the two groups after treatment.Results:The degree of wrist swelling regression,visual analogue scale(VAS)score,the evaluation of callus growth time and the evaluation of wrist joint function in the observation group was superior to that of the control group(P<0.05),while there was no significant difference between the two groups in the relation of wrist anatomical structure(P>0.05),but it was significantly better than before treatment(P<0.01),and there was no statistical difference until after 12 weeks(P<0.05).Compared with before treatment,the levels of osteocalcin and the amino terminal of serum total type Ⅰ collagen in both groups were increased after treatment,and the changes in the observation group were more significant than those in the control group(P<0.05).Conclusion:Manual reduction and small splint fixation can restore and maintain the normal anatomical relationship of the wrist joint,and it combined with the “snake difficult to climb” anti-swelling powder modified prescription external application can significantly reduce the pain and swelling of the fracture,improve bone metabolism,and promote fracture healing in elderly patients.
