摘要:
目的:观察改良拔河针法对膝骨关节炎(KOA)兔关节软骨修复的作用。方法:18只健康新西兰家兔随机分为对照组、关节炎组和改良拔河针治疗组(治疗组),每组各6只。采用膝关节腔注射木瓜蛋白酶诱导膝骨关节炎模型。治疗组实验兔采用改良拔河针法进行干预,1次/d,10 d为1个疗程,每个疗程间隔3 d,共治疗2个疗程。对照组和膝骨关节炎组不进行任何治疗。采用Lequesne 行为学评分对各组实验兔进行行为学评价; 采用苏木精-伊红(HE)染色及番红固绿染色观察各组实验兔膝关节软骨组织形态学变化; 采用酶联免疫吸附(ELISA)法检测血清和滑膜液中白介素(IL)-1β和肿瘤坏死因子(TNF)-α; RT-qPCR实验检测软骨组织中NF-κB、NLRP3、Caspase-1 mRNA的表达。结果:与对照组相比,膝骨关节炎模型兔的行为学评分显著升高,左膝关节周径和直径显著增加,差异有统计学意义(P<0.01); 而经改良拔河针治疗后,膝骨关节炎兔行为学评分明显降低,左膝关节周径和直径均显著缩小,差异有统计学意义(P<0.01)。HE染色和番红固绿染色结果显示,改良拔河针能显著减轻膝骨关节炎兔关节软骨的病理损伤。同时,与膝骨关节炎组相比,治疗组滑膜液和血清中IL-1β和TNF-α的水平显著降低,差异有统计学意义(P<0.01)。改良拔河针法能抑制膝骨关节炎兔软骨组织中NF-κB、NLRP3、Caspase-1 mRNA的表达,差异有统计学意义(P<0.01)。结论:改良拔河针法可通过抑制炎症因子促进膝骨关节炎兔软骨修复,其作用机制可能与NF-κB/NLRP3/Caspase-1炎症小体信号通路有关。
Abstract:
Objective:To observe the role of modified Bahe Acupuncture in the repair of articular cartilage in knee osteoarthritis(KOA)rabbits.Methods:18 healthy New Zealand rabbits were randomly divided into a control group,a model group(KOA group)and a modified Bahe Acupuncture treatment group,with 6 rabbits in each group.The KOA model was induced by papain injection into the knee joint cavity.Rabbits in the treatment group were treated with the modified Bahe Acupuncture once a day,10 d was a treatment course,and each course was separated by 3 d for a total of 2 treatment courses.No treatment was given in the control and KOA groups.Behavioral evaluation of the experimental rabbits in each group was performed using the Lequesne score; morphological changes of knee cartilage histology were observed by hematoxylin-eosin(HE)staining and safranin O-fast green staining; the expression levels of IL-1β and TNF-α in serum and synovial fluid were detected by enzyme-linked immunosorbent assay(ELISA); RT-qPCR assay was used to detect the expression of NF-κB,NLRP3 and Caspase-1 mRNA in cartilage tissues.Results:Compared with the control group,the behavioral scores of rabbits in the KOA group were significantly higher,and the circumference and diameter of the left knee joint were significantly increased(P<0.01); whereas,aftermodified Bahe Acupuncture treatment,the behavioral scores of KOA rabbits were significantly lower,and the circumference and diameter of the left knee joint were significantly reduced(P<0.01).Meanwhile,the levels of IL-1β and TNF-α in serum and synovial fluid were significantly lower in the treatment group compared with the KOA group(P<0.01).In addition,modified Bahe Acupuncture treatment could inhibit the expression of NF-κB,NLRP3 and Caspase-1 mRNA in cartilage tissue of KOA rabbits(P<0.01).Conclusion:The modified Bahe Acupuncture method can effectively promote cartilage repair in KOA rabbits by inhibiting inflammatory factors,and its mechanism of action may be related to NF-κB/NLRP3/Caspase-1 signaling pathway.
