摘要:
目的:观察直接外侧入路行人工股骨头置换术对高龄股骨颈骨折患者髋关节功能及生存质量的影响。方法:回顾性分析2019年1月至2022年6月采用直接外侧入路行人工股骨头置换术的53例高龄股骨颈骨折患者,记录手术时间、术中出血量、术后引流量、下床活动时间、完全负重时间,并分别于术前、术后1个月、3个月、6个月记录患者的髋关节Harris评分及SF-36量表评分,评定术后髋关节功能及生存质量的恢复情况。结果:53例患者均获得随访,手术时间为(61.1±7.2)min,术中出血量为(282.8±26.5)mL,术后引流量为(194.1±23.2)mL,术后下床活动时间为(2.87±0.7)d,完全负重时间为(6.9±2.4)d。术后1个月Harris评分为(78.0±5.7)分,SF-36评分为(64.7±8.3)分,均比术前评分升高,差异有统计学意义(P<0.01)。术后3个月Harris评分为(83.1±5.8)分,SF-36评分为(78.5±6.6)分,均比术后1个月有显著升高,差异有统计学意义(P<0.05)。术后6个月Harris评分为(84.1±5.2)分,SF-36评分为(79.5±6.0)分,与术后3个月相比差异无统计学意义(P>0.05)。53例患者在随访中均无出现脱位、感染等并发症。结论:直接外侧入路行人工股骨头置换术对高龄股骨颈骨折患者髋关节功能及生存质量均有显著疗效,术后早期疗效好,恢复快,且并发症少,值得临床推广应用。
Abstract:
Objective:To observe the efficacy of artificial femoral head replacement through the direct lateral approach on hip joint function and quality of life in elderly patients with femoral neck fracture.Methods:A retrospective analysis was conducted on 53 elderly patients with femoral neck fracture who underwent artificial femoral head replacement through the direct lateral approach from January 2019 to June 2022.The study recorded the operation time,intraoperative blood loss,postoperative drainage volume,ambulation time,and full weight-bearing time.The harris hip Score and SF-36 scale score were assessed before the operation,as well as at 1 month,3 months,and 6 months after the operation to evaluate the recovery of hip function and quality of life.Results:All 53 patients were successfully followed up.The average operation time was(61.1±7.2)min,intraoperative blood loss was(282.8±26.5)mL,postoperative drainage volume was(194.1±23.2)mL,ambulation time was(2.87±0.70)d,and full weight-bearing time was(6.9±2.4)d.At 1 month after the operation,the harris hip score was(78.0±5.7)points,and the SF-36 scale score was(64.7±8.3)points,both of which were significantly higher than the preoperative scores(P<0.01).The harris hip score((83.1±5.8)points)and SF-36 scale score((78.5±6.6)points)at 3 months after the operation remained significantly higher than the 1 month postoperative scores(P<0.05).The Harris Hip Score and SF-36 scale score at 6 months after the operation were(84.1±5.2)points and(79.5±6.0)points,respectively,without significant differences compared to the scores at 3 months after the operation(P>0.05).No cases of dislocation,infection,or other complications were reported during the follow-up period.Conclusion:Artificial femoral head replacement through the direct lateral approach has a significant positive impact on hip joint function and quality of life in elderly patients with femoral neck fracture.The procedure shows good early postoperative outcomes,rapid recovery,and few complications,indicating its potential for clinical application.
