摘要:
目的:比较分析富血小板血浆联合股骨头坏死愈胶囊治疗不同面积股骨头坏死的疗效。方法:回顾性分析 2018年8月至2020年6月接受富血小板血浆联合股骨头坏死愈胶囊治疗的80例(109髋)ARCOⅡ期非创伤性股骨头坏死患者的病例资料,运用股骨头坏死面积统计软件评估所有患髋治疗前的坏死面积比例,并将其分为两组,坏死面积比例≤30%组39例(54髋),坏死面积比例>30%组41例(55髋); 比较两组患者治疗后6个月及末次随访时的临床疗效与影像资料。结果:两组患者随访时间均为18个月以上,平均为(25.48±8.48)个月。治疗后6个月时,坏死面积比例≤30%组在疼痛视觉模拟量表(VAS)评分、Harris 评分及髋伸屈范围等方面显著优于坏死面积比例>30%组,差异有统计学意义(P<0.05); 末次随访时,坏死面积比例≤30%组VAS 评分、Harris 评分以及髋伸范围、内外旋范围、内收外展范围均显著优于坏死面积比例>30%组,差异有统计学意义(P<0.05)。末次随访时两组的影像评级、骨髓水肿分级差异均有统计学意义(P<0.05); 随访过程中所有病例均未出现严重不良反应。结论:富血小板血浆联合股骨头坏死愈胶囊可明显改善ARCOⅡ期股骨头坏死的症状,改善其髋关节功能,对小面积坏死者(坏死面积比例≤30%)效果更优。
Abstract:
Objective:To compare and analyze the efficacy of platelet-rich plasma combined with Gugutou Huaisiyu capsules on the treatment of osteonecrosis of the femoral head with different lesion areas.Methods:The clinical data of 80 patients(109 hips)with early and middle stage non-traumatic osteonecrosis of the femoral head treated with platelet-rich plasma combined with Gugutou Huaisiyu capsules from August 2018 to June 2020 were analyzed retrospectively. The area ratio of necrosis of all hips before treatment was evaluated by femoral head necrosis area statistical software, and the patients were divided into two groups: 39 cases(54 hips)in area ratio of necrosis ≤30% group, 41 cases(55 hips)in area ratio of necrosis >30% group. The clinical efficacy and imaging data of the two groups were compared 6 months after treatment and at the last follow-up.Results:The follow-up time of patients in both groups was more than 18 months, with an average of(25.48 ±8.48)months. At 6 months after treatment, the visual analogue scale(VAS)score, Harris score and range of motion(ROM)of hip extension and flexion in the area ratio of necrosis ≤30% group were significantly better than those in the area ratio of necrosis >30% group(P<0.05). At the last follow-up, the VAS score, Harris score, hip extension ROM, internal and external rotation ROM and adduction and abduction ROM in the area ratio of necrosis ≤30% group were significantly better than those in the area ratio of necrosis >30% group(P>0.05). In terms of imaging, there were significant differences in imaging rating and bone marrow edema grade between the two groups during the last follow-up, and there were no serious adverse reactions in all cases during the follow-up.Conclusion:Platelet-rich plasma combined with Gugutou Huaisiyu capsules can significantly improve the symptoms of early and middle osteonecrosis of the femoral head, improve the function of the hip joint, and has a better efficacy on small area(area ratio of necrosis ≤30%)necrotic deaths.
