摘要:
目的:分析专家与初学者操作坐位腰椎旋转手法时的运动学与力学特征。方法:招募20例健康受试者,年龄为18~40岁。由1名有10年以上手法经验的专家与10名中医骨伤科专业在读博士生(初学者组)对20例受试者行坐位腰椎旋转手法操作,应用运动捕捉技术与测力手套对手法操作时的运动学与力学数据(运动学参数:旋扳过程中的最大速度,最大加速度; 旋扳过程中旋转、前屈、侧屈的最大角度。力学参数:旋扳过程中拇指最大顶推力,最大掌侧力,最大力矩等)进行测量,总结坐位腰椎旋转手法的运动学与力学特征。结果:两组数据相比较,初学者手法操作时旋扳最大速度与旋扳最大加速度显著小于专家操作(P<0.05),旋转最大角度显著大于专家操作(P<0.05); 初学者手法操作时拇指最大顶推力、旋扳最大掌侧力、最大力矩等参数均显著小于专家操作(P<0.05)。结论:初学者坐位腰椎旋转手法操作过程中的常见问题是旋转幅度过大、发力偏小、扳动速度慢。初学者应注重该手法扳动时力、速度、幅度的控制,达到高速低幅的扳动要求。
Abstract:
Objective:To analyze and compare the kinematic and mechanical characteristics of practitioners with different experience when operating the sitting lumbar spine manipulation.Methods:20 healthy subjects were recruited.The age was 1840 years old.The manipulation of sitting lumbar rotation was performed on 20 subjects by an expert with more than 10 years of manipulation experience and 10 doctoral students(beginners)majoring in orthopedics and traumatology of traditional Chinese medicine.The kinematic and mechanical data(kinematic parameters:maximum velocity and acceleration during rotation; maximum angle of rotation,forward flexion and lateral flexion during rotation.Mechanical parameters:maximum thumb thrust,maximum palmar force,maximum torque,etc.)were measured by using motion capture technology and force measuring gloves during the rotation.The kinematic and mechanical characteristics of the sitting lumbar spine manipulation were summarized.Results:Compared with the two groups of data,the maximum speed and the maximum acceleration of the rotary trigger during the beginners' manipulation were significantly smaller than those of the experts(P<0.05),and the maximum angle of rotation was significantly larger than those of the experts(P<0.05); the maximum thumb thrust,the maximum palm side force and the maximum moment of the rotary trigger during the beginners' manipulation were significantly smaller than those of the experts(P<0.05).Conclusion:The common problems during the operation of the sitting lumbar spine manipulation for beginners are excessive rotation amplitude,small force and slow triggering speed.Beginners should pay attention to the control of force,speed and amplitude during the manipulation to achieve the requirement of high speed and low amplitude of manipulation.
