摘要:
目的:评估平均坏死面积指数(MNAI)和平均稳定指数(ASI)与坏死股骨头塌陷进展的相关性。方法:回顾性分析2013年8月至2020年10月接受非手术保髋治疗的148例(220髋)股骨头坏死塌陷前患者的病例资料,根据末次随访时股骨头塌陷情况分为塌陷组和非塌陷组,其中塌陷组83例124髋,非塌陷组65例96髋。根据所有纳入病例初次就诊时的磁共振成像(MRI)资料计算出坏死股骨头的平均坏死面积指数和平均稳定指数,利用非参数检验评估该指数在两组之间的差异性,利用受试者工作特征(ROC)曲线评价该指数预测塌陷进展的特异性与敏感性。结果:非参数检验结果表明,塌陷组平均坏死面积指数(5.63±2.18)高于非塌陷组(2.82±1.34),差异有统计学意义(Z=-9.112,P<0.001); 塌陷组平均稳定指数(2.20±1.72)低于非塌陷组(6.88±2.53),差异有统计学意义(Z=-10.703,P<0.001)。平均坏死面积指数的受试者工作特征曲线下面积为0.920,临界值为5.595,灵敏度为76.0%,特异度为96.8%; 平均稳定指数的受试者工作特征曲线下面积为0.858,临界值为4.300,灵敏度为72.6%,特异度为85.4%。结论:平均坏死面积指数和平均稳定指数与股骨头塌陷进展之间有相关性,可以作为评估股骨头坏死塌陷的重要参考指标。
Abstract:
Objective:To evaluate the correlation between mean necrotic area index(MNAI),average stability index(ASI)and the progression of necrotic femoral head collapse.Methods:The medical records of 148 patients(220 hips)who received non-surgical hip preservation treatment from August 2013 to October 2020 were retrospectively analyzed.According to the situation of femoral head collapse during the last follow-up,the patients were divided into collapse group(124 hips)and non-collapse group(96 hips).The MNAI and ASI of necrotic femoral head were calculated according to the MRI data of all cases at the first visit.Nonparametric test was used to evaluate the difference of the index between the two groups,and receiver operating characteristic(ROC)curve was used to evaluate the specificity and sensitivity of the index in predicting collapse progress.Results:The results of nonparametric test showed that the MNAI of collapse group(5.63±2.18)was significantly higher than that of non-collapse group(2.82±1.34)(P<0.001).The ASI in the collapse group(2.20±1.72)was significantly lower than that in the non-collapse group(6.88±2.53)(P<0.001).The area under ROC curve of MNAI is 0.920,the critical value is 5.595,the sensitivity is 76.0%,and the specificity is 96.8%.The area under the ROC curve of ASI is 0.858,the critical value is 4.300,the sensitivity is 72.6%,and the specificity is 85.4%.Conclusion:There is a correlation between MNAI,ASI and the progression of femoral head collapse,which can be used as an important reference index to evaluate the collapse of femoral head necrosis.
