摘要:
目的:探讨仲黄颗粒对老年性骨质疏松椎体压缩性骨折(Osteoporotic Vertebral Compression Fracture,OVCF)椎体成形术后患者骨转换标志物及生活质量(Quality of Life,QOL)的影响。方法:选取自2017年1月至2019年12月,在本院诊疗行椎体(后凸)成形术的老年性OVCF患者90例,随机分为治疗组和对照组(治疗组采用仲黄颗粒治疗,对照组采用仙灵骨葆治疗)。疗程均为6个月,观察骨密度、疼痛视觉模拟评分(VAS)、血清Ⅰ型原胶原氨基端前肽(PINP)、血清骨钙素(OST)、血清β-胶联降解产物(β-CTx)、生活质量问卷(QUALEFFO-41量表)评分变化。结果:治疗组和对照组分别有41例和38例患者获得随访,两组患者对临床疗效均较为满意。对比治疗前,两组患者骨密度、VAS评分、PINP、QUALEFFO-41量表评分及治疗组OST、β-CTx均显著改善,差异有统计学意义(P<0.05); 对比对照组,治疗组6个月VAS评分和PINP改善更为明显,组间差异有统计学意义(P<0.05)。结论:仲黄颗粒和仙灵骨葆均可以提高老年OVCF术后患者骨密度、减轻腰背疼痛、改善生活质量,与仙灵骨葆相比,仲黄颗粒能更有效缓解疼痛、调节骨代谢和促进成骨作用。
Abstract:
Objective:To observe the efficacy of bone turnover marker and quality of life(QOL)of Zhong Huang granule on the treatment of senile OVCF(osteoporotic vertebral compression fracture)with postoperative vertebroplasty.Methods:90 cases with senile OVCF treated by postoperative vertebroplasty were selected and randomed into treatment group and control group from January 2017 to December 2019.Patients on the treatment group were treated by Zhong Huang granule,in the control group,patients were treated by Xianling Gubao.Bone mineral density,VAS scores,aminoterminal prepeptide typeⅠprocollagen(PINP),serum osteocalcin(OST),β-C-terminal telopeptide of type Ⅰ collagen(β-CTx),and QUALEFFO-41 were observed and recorded before and 6 months after treatment.Results:41 patients in the treatment group were followed up,and 38 patients in the control group were followed up.Satisfactory clinical efficacy was achieved in both groups.Bone mineral density,VAS scores,PINP,QUALEFFO-41 scores in both groups,and OST,β-CTx on the treatment group were significantly improved after treating(P<0.05).The VAS scores and PINP in the treatment group were significantly improved compared with the control group(P<0.05).Conclusion:Both Zhong Huang granule and Xianling Gubao can improve bone density,relieve back pain and improve the living quality.Zhong Huang granule can relieve pain,regulate bone metabolism and promote osteogenesis more effectively than Xianling Gubao granule.
