摘要:
目的:观察术中超声引导结合C臂机透视单侧经皮椎体成形术(PVP)治疗上中胸椎骨质疏松性骨折的临床疗效。 方法:回顾性分析2018年7月至2021年3月应用超声加C臂机透视引导单侧经皮椎体成形术治疗上中胸椎骨质疏松性骨折27例(共33节段胸椎)患者资料。其中男3例,女24例; 年龄56~88岁,平均72岁。所有患者术前均无神经根性症状。通过手术时间、术中透视次数、住院时间,术前、术后的疼痛VAS评分,骨水泥渗漏及术后早期并发症,评定其临床疗效。 结果:术后疼痛改善明显,本组27例均获随访,平均随访时间9个月(6~12个月)。患者均未出现伤口感染、神经根症状、气胸、栓塞等并发症。 结论:术中超声加C臂机透视引导单侧经皮椎体成形术治疗上中胸椎骨质疏松性骨折,能够减少透视次数,精准、安全、有效,值得推广应用。
Abstract:
To investigate the clinical efficacy of ultrasound assisted combine with C-arm fluoroscopic unilateral PVP in treating upper and mid-thoracic osteoporotic fractures. Methods:27 cases(33 segments)of upper and mid-thoracic osteoporotic vertebral compression fractures from July 2018 to March 2021 were analyzed retrospectively, including 3 male and 24 female. The age ranged from 56 to 88 years old, with 72 years old on average. All patients had no symptoms of nerve root before operation. Operation time, intraoperative fluoroscopies times, hospital stay, visual analogue score(VAS)before and after operation, bone cement leakage and postoperative complications were compared to evaluated the clinical efficacy. Results:Postoperative pain improved significantly. All patients were followed up from 6 to 12 months, with 9 months on average. There were no complications such as wound infection, nerve root symptom, pneumothorax and hematoma. Conclusion:The ultrasound assisted combine with C-arm fluoroscopic unilateral PVP on the treatment of upper and mid-thoracic osteoporotic fractures can significantly reduce the times of fluoroscopy. This technique is accurate, safe and effective which is worth popularizing.
