摘要:
目的:探讨肩关节镜下单纯清理钙化灶,对于受累肌腱不加以缝合的手术方案治疗冈上肌钙化性肌腱炎的临床疗效。方法:回顾分析2019年1月至2020年12月收治的冈上肌腱钙化性肌腱炎患者13例,男4例,女9例; 年龄35～68岁,中位数54岁; 左侧5例,右侧8例。术中均行关节镜下钙化灶清理,未缝合。术前、术后及后期复查均采用视觉模拟评分法(VAS)疼痛评分、Constant-Murley 评分及满意度评估来评价患者恢复情况。结果:13例患者术后平均随访6.4～10.8个月,症状均明显改善,VAS疼痛评分由术前(8.2±1.4)分至术后 6 个月(0.8±0.6)分,Constant-Murley 评分由术前(43.2±7.4)分至术后 6 个月(85.8±5.6)分,差异均有统计学意义(P<0.05)。结论:关节镜下单纯清理钙化灶,不缝合肌腱,是一种治疗冈上肌腱钙化性肌腱炎的有效方法,手术损伤较小,患者满意度较高。
Abstract:
Objective:To investigate the clinical efficacy simple cleaning of calcified focus and no suture of the affected tendon under arthroscopy for calcified tendonitis of supraspinatus tendon.Methods:13 patients with supraspinatus tendonitis with calcified tendonitis admitted from January 2019 to December 2020 were retrospectively analyzed.There were 4 males and 9 females ranging in age from 35 to 68 years old with a median age of 54 years old.There were 5 cases on the left side and 8 cases on the right.Arthroscopic clearance of calcification was performed without suture during the operation.Visual analogue score,Constant-Murley score and satisfaction evaluation were used to evaluate the recovery of patients before and after surgery and later review.Results:The 13 patients were followed up for 6.4 to 16.8 months on average, and their symptoms improved significantly,visual analogue scores ranged from(8.2±1.4)scores preoperatively to(0.8±0.6)scores 6 months postoperatively, Constant-Murley scores ranged from(43.2±7.4)scores preoperatively to(85.8±5.6)scores 6 months postoperatively,the differences were statistically significant(P<0.05).Conclusion:It is an effective method to treat supraspinatus tendonitis by simply cleaning the calcified focus without suturing the tendon under arthroscopy.This method has less surgical injury and higher patient satisfaction.
