摘要:
目的:对零切迹颈椎前路椎间融合固定系统(Zero-P)治疗颈椎病的临床疗效进行评估。方法:统计2017年1月至2018年12月行颈椎前路Zero-P固定的24例颈椎病患者。记录手术时间,术中出血量,颈椎融合及内固定情况,神经功能及生活质量评估,术后并发症发生情况。结果:所有患者平均手术时间(65.2±17.1)min,出血量(41.4±20.3)mL。神经功能改善情况EMS评分:术前(9.04±2.12)分,术后1周(14.25±3.47)分,术后3个月(16.22±2.16)分,术后6个月(16.92±1.55)分,术后12个月(17.15±1.07)分。生活质量状况BI量表:术前(57.28±19.02)分,术后1周(50.51±22.36)分,术后3个月(59.66±17.42)分,术后6个月(80.52±24.05)分,术后12个月(90.20±11.58)分。术后随访,术后1周出现1例中度(根据Bazaz 评分)吞咽困难,术后1个月未出现吞咽困难; 术后12个月椎间融合率100%,未出现邻椎病情况; 随访中未出现脑脊液漏、切口感染、喉返神经损伤、内固定移位等并发症。结论:零切迹椎间融合内固定 Zero-P 系统治疗颈椎病操作简便、疗效显著,术后并发症发生率相对较低,值得临床推广应用。
Abstract:
To evaluate the clinical efficacy of zero-notch anterior cervical interbody fusion and fixation system(Zero-P)in the treatment of cervical spondylosis.Methods:24 cases cervical spondylosis patients treated by cervical anterior cervical Zero-P fixation were analyzed from January 2017 to December 2018 in our department.The operative time,intraoperative bleeding,cervical fusion of internal fixation,neurological function and postoperative complications were recorded.Results:The average operation time was(65.2±17.1)min.The amount of bleeding was(41.4±20.3)mL.The neurological function of EMS scores were preoperatively improved from 9.04±2.12 to 14.25±3.47 at 1st week,16.22±2.16 at 3rd month,16.92±1.55 at 6th month and 17.15±1.07 at 12th month after surgery.The neurological function of EMS scores were preoperatively improved from 57.28±19.02 to 50.51±22.36 at 1st week,59.66±17.42 at 3rd month,16.92±1.55 at 6th month and 90.20±11.58 at 12th month after surgery.One patient had moderate dysphagia(according to Bazaz score)one week after operation and showed no dysphagia one month later.During the follow-up interbody fusion rate was 100%,and no adjacent vertebral disease CSF fistula,incision infection,injury of recurrent laryngeal nerve and internal fixation shift happened.Conclusion:Internal fixation with Zero-P system for cervical spondylosis is a simple and effective method with relatively low postoperative complications.
