摘要:
目的:观察取自体带骨膜髂骨配合软骨下骨板钻孔并打压植骨改良术治疗Hepple Ⅴ型距骨骨软骨损伤的手术技巧与临床疗效。方法:对13例采用骨膜骨移植配合软骨下骨板钻孔并打压植骨治疗Hepple Ⅴ型距骨骨软骨损伤的患者进行回顾性分析,于末次随访时行MRI或CT检查并评价患者术后AOFAS(美国足踝外科协会踝与后足评分)、VAS评分(视觉模拟评分法)、患者跖屈与背伸角度以及病人满意度。结果:除1例患者骨膜增殖异常,需关节镜二次清理外,其余患者踝关节疼痛均消失,功能活动正常,术后均无供体区疼痛现象,患者满意度85.7%。AOFAS自术前(43.15±7.95)分显著上升至(82.85±13.39)分,VAS评分自(4.77±0.93)分下降至(2.00±1.47)分,跖屈角度自术前的21.89°±7.36°升至28.38°±6.63°,背伸角度自术前的15.92°±7.09°升至20.77°±4.90°,差异均有统计学意义(P<0.05)。结论:改良带骨膜骨移植手术治疗Hepple Ⅴ型距骨骨软骨损伤疗效确切,并可减轻供体区疼痛。
