摘要:
目的:比较低黏度经皮椎体后凸成形术(PKP)与高黏度经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩骨折的疗效。方法:选取2015年4月至2018年6月收治的66例单椎体骨质疏松性压缩骨折患者为研究对象,按照简单随机分组方法,将其分为对照组(低黏度骨水泥PKP)和研究组(高黏度骨水泥PVP),两组各33例。比较两组患者手术时间、透视次数、骨水泥渗漏率、骨水泥量和手术前后椎体后凸Cobb角、前缘高度、视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)的差异。结果:研究组骨水泥渗漏率和骨水泥量明显低于对照组,差异均有统计学意义(P<0.01)。研究组手术时间和透视次数明显少于对照组,差异均有统计学意义(P<0.01)。两组患者术后1周和4周VAS评分与术前比较,差异均有统计学意义(P<0.01); 研究组术前1 d、术后1周和4周VAS评分与对照组比较,差异均无统计学意义(P>0.05)。两组患者术后3个月和12个月ODI值与术前比较,差异均有统计学意义(P<0.01); 研究组术前1 d、术后3个月和12个月ODI值与对照组比较,差异均无统计学意义(P>0.05)。研究组术后椎体后凸Cobb角和椎体前缘高度与术前相比,差异均无统计学意义(P>0.05)。对照组术后椎体后凸Cobb角和椎体前缘高度与术前相比,差异均有统计学意义(P<0.01)。两组间术前及术后的椎体后凸Cobb角和椎体前缘高度相比,差异均无统计学意义(P>0.05)。结论:高黏度骨水泥PVP在骨质疏松性椎体压缩骨折治疗中应用价值较高,值得推广应用。
Abstract:
Objective:To compare the efficacy of low-viscosity percutaneous kyphoplasty(PKP)and high-viscosity percutaneous vertebroplasty(PVP)in treating osteoporotic vertebral compression fractures.Methods:66 patients with osteoporotic compression fractures of single vertebral body admitted to our hospital from April 2015 to June 2018 were selected as the research objects.According to the simple random grouping method,they were divided into control group(low-viscosity bone cement PKP)and research group(high-viscous bone cement PVP)with 33 cases in each group.The operation time,fluoroscopy times,cement leakage rate,cement volume and Cobb angle,anterior margin height,visual analogue scale(VAS),Oswestry dysfunction index(ODI)of kyphosis before and after surgery were compared between the two groups.Results:The leakage rate and amount of bone cement in the study group were significantly lower than those in the control group(P<0.01).The operation time and fluoroscopy times in the study group were significantly less than those in the control group,and the statistically significant difference was found(P<0.01).The VAS scores of the two groups at 1 week and 4 weeks after operation were significantly different from those before operation(P<0.01); the VAS scores of the study group at 1 d before operation,1 week after operation and 4 weeks after operation were not significantly different from those of the control group(P>0.05).The ODI of the two groups at 3 and 12 months after operation were significantly different from those before operation(P<0.01); the ODI of the study group at 1 d before operation,3 and 12 months after operation were not significantly different from those of the control group(P>0.05).There was no significant difference in Cobb angle of kyphosis and anterior height of vertebral body before and after surgery in research group(P>0.05).The Cobb angle of kyphosis and anterior height of vertebral body in control group were significantly different from those before operation(P<0.01).There was no significant difference in Cobb angle of kyphosis and anterior vertebral height between the two groups before and after operation(P>0.05).Conclusion:High-viscous bone cement PVP has high application value in the treatment of osteoporotic vertebral compression fracture,and it is worth popularizing.
