摘要:
目的:探讨尺骨正向变异与老年女性柯莱斯骨折中医手法复位后功能恢复的关系。方法:收集126例经中医传统手法复位石膏外固定治疗后的老年女性柯莱斯骨折患者相关资料,待12周骨折完全愈合后,在标准后前位X线片上测量尺骨头关节面平行线与桡骨乙状切迹最远端关节面平行线之间的位置距离差,并以其位置距离差>2 mm为阳性变异(即正向变异)。依据Cooney腕关节评分将患者分为优良组(评分≥80分)和中差组(评分<80分),分析尺骨正向变异与Cooney腕关节评分的关系。结果:优良组86例,中差组40例。经统计学分析,发现尺骨正向变异与老年女性柯莱斯骨折中医手法复位后腕关节功能优良率负相关(B=-0.820<0),且差异有统计学意义(P=0.04<0.05)。结论:尺骨正向变异是老年女性柯莱斯骨折中医手法复位后功能恢复的重要因素,在临床行手法复位石膏外固定患者的治疗中需要对这一解剖结构的恢复予以足够的关注。
Abstract:
Objective:To study the association between ulnar positive variance and the function of Colles' fractures in elderly women after traditional Chinese manual reduction.Methods:To collect the data of Colles'fracture in elderly women after traditional Chinese manual reduction and plaster external fixation.The fracture healed completely after 12 weeks.On a standard posterior-anterior X-ray,the distance between the parallel lines on the articular surface of the ulnar bone and the radial'sigmoid incisures have been mesured.If the range difference was more than 2 mm,we have defined it as ulnar positive variance.According to Cooney wrist joint score,they were divided into two groups:the well group(>80)and the ill group(<80).Finally,the relationship between ulnar positive variance and Cooney wrist joint score was analyzed.Results:There were 86 cases in the well group and 40 cases in the ill group.Using statistical analysis,they showed a negative correlation(B=-0.820<0)between ulnar positive variance and the function of Colles' fractures in elderly women after traditional Chinese manual reduction,and there was no significance in statistics(P=0.04<0.05).Conclusion:Ulnar positive variance is an important factor in the function of Colles' fractures in elderly women after traditional Chinese manual reduction.In the treatment of patients with external plaster fixation by traditional Chinese manual reduction,doctors should pay adequate attention to the restoration of this anatomical structure.
