摘要:
目的:分析聚蛋白多糖酶-4(ADAMTS-4)及基质金属蛋白酶-3(MMP-3)在骨关节炎(OA)中的表达,探讨两者在OA的发生和进展中的作用。方法:收集40例OA标本(关节软骨、关节滑膜、关节液)及40例正常软骨、关节滑膜及关节液标本。关节软骨、关节滑膜标本中的ADAMTS-4及MMP-3表达采用免疫组化法测定,关节液标本中的ADAMTS-4及MMP-3采用酶联吸附法(ELISA)测定。比较OA关节和正常关节的关节软骨、OA关节液和正常关节液中的ADAMTS-4及MMP-3的产物表达情况。比较ADAMTS-4及MMP-3在关节滑膜中的表达。结果:在正常软骨组织和OA软骨组织中,存在ADAMTS-4及MMP-3的表达。ADAMTS-4主要表达于软骨细胞胞浆内,可见大量棕黄色颗粒、细胞膜黄染的细胞,特别是固缩退变的软骨细胞更明显; MMP-3的表达主要在软骨细胞胞浆内,细胞内可见棕褐色颗粒。正常软骨中ADAMTS-4阳性表达量较少,且主要表达在表层,深层无表达; OA软骨中ADAMTS-4阳性表达量较正常软骨明显升高,差异有统计学意义(P<0.05),尤其是表层表达量升高量更明显,其表达量随软骨深度的增加而降低(P<0.05)。正常软骨除了表层少量表达MMP-3外,中层、深层均无表达; OA软骨中各层MMP-3表达量明显升高,差异有统计学意义(P<0.05),且随软骨深度的增加而升高,差异有统计学意义(P<0.05)。在正常滑膜组织和OA滑膜组织中,均存在ADAMTS-4及MMP-3的表达,但分布无明显规律。ADAMTS-4在滑膜组织中呈浅黄色,多定位于细胞胞浆; MMP-3在滑膜组织中呈棕色,在OA患者滑膜组织中可见大量棕色颗粒分布在细胞胞浆。ADAMTS-4及MMP-3在滑膜中的阳性表达率差异有统计学意义(P<0.05)。OA关节液的ADAMTS-4和MMP-3的产物ARGxx和FFGxx的表达水平均较正常关节液明显升高,差异有统计学意义(P<0.05)。结论:在人的OA中,ADAMTS-4,MMP-3在OA的发生和进展中均发挥作用。磨损的关节软骨均高表达ADAMTS-4和MMP-3,且随着磨损程度的加深,ADAMTS-4表达水平逐渐减少,而MMP-3表达水平逐渐增加。抑制ADAMTS-4及MMP-3的活性可能是OA防治的一个治疗方向。
Abstract:
Objective:To analyze the expression of polyproteoglycan-4(ADAMTS-4)and matrix metalloproteinase-3(MMP-3)in osteoarthritis(OA)and explore their roles in the occurrence and progress of OA.Methods:40 OA specimens(articular cartilage,synovium and synovial fluid)and 40 normal cartilage,synovium and synovial fluid specimens were collected.The expressions of ADAMTS-4 and MMP-3 in articular cartilage and synovium were detected by immunohistochemical method.The expressions of ADAMTS-4 and MMP-3 in synovial fluid were determined by enzyme-linked sorption assay(ELISA).The expression of ADAMTS-4 and MMP-3 in cartilage,OA synovial fluid and normal synovial fluid of OA and normal joints were compared.To compare the expression of ADAMTS-4 and MMP-3 in synovium.Results:There were expressions of ADAMTS-4 and MMP-3 in normal cartilage tissue and OA cartilage tissue.ADAMTS-4 was mainly expressed in the cytoplasm of chondrocyte,and a large number of brown-yellow granules and yellowish-stained cell membranes were observed,especially in the chondrocyte with solid degeneration.MMP-3 was mainly expressed in the cytoplasm of chondrocyte,and brown granules were observed in the cells.The positive expression of ADAMTS-4 was less in normal cartilage,and mainly in the surface layer,but not in the deep layer.The positive expression of ADAMTS-4 in OA cartilage was significantly higher than that in normal cartilage(P< 0.05),especially in the surface layer.The expression of ADAMTS-4 decreased with the increase of cartilage depth(P< 0.05).There was no expression of MMP-3 in the middle and deep layers of normal cartilage except for a little expression of MMP-3 in the surface layer.The expression of MMP-3 in all layers of OA cartilage increased significantly(P< 0.05),and increased with the increase of cartilage depth(P< 0.05).ADAMTS-4 and MMP-3 were expressed in normal synovial tissues and OA synovial tissues,but the distribution of ADAMTS-4 and MMP-3 was not obvious.ADAMTS-4 was light yellow in synovial tissue,mostly located in cytoplasm; MMP-3 was brown in synovial tissue,and a large number of brown granules were found in synovial tissue of patients with OA.The positive rates of ADAMTS-4 and MMP-3 in synovium were significantly different(P< 0.05).The expression levels of ARGxx and FFGxx in OA synovial fluid were significantly higher than those in normal synovial fluid(P< 0.05).Conclusion:In human OA,ADAMTS-4 and MMP-3 play an important role in the occurrence and progress of OA.The expression of ADAMTS-4 and MMP-3 in worn articular cartilage was high.With the deepening of wear,the expression of ADAMTS-4 decreased gradually,while the expression of MMP-3 increased gradually.Inhibiting the activities of ADAMTS-4 and MMP-3 may be one of the therapeutic approaches for prevention and treatment of OA.
