摘要:
目的:探讨经同侧距骨非负重面获取骨软骨柱修复距骨软骨损伤的手术技巧及疗效。方法:回顾性分析2016年6月至2018年12月采用自体同侧距骨非负重面软骨柱移植修复HeppleⅢ~Ⅴ型距骨骨软骨损伤76例。术前、术后分别对患者采用Karlsson踝关节功能评分(KAFS)、视觉疼痛量表(VAS)、美国足与踝协会踝与后足评分(AOFAS)及VASFA评分来评估手术效果和功能状况。术后1年MRI检查植骨处骨软骨愈合情况。结果:术后均得到6~24个月随访,平均15个月。利用标准VAS评估踝关节疼痛程度,评分从术前平均(6.9±2.8)分降至术后(1.6±1.8)分,差异有统计学意义(P<0.001); AOFAS评分由术前的(77.32 ± 6.67)分提高到术后的(93.10 ± 8.24)分,差异有统计学意义(P<0.001); Karlsson评分由术前平均(52.16±9.85)分上升至(85.73±11.18)分,差异有统计学意义(P<0.001)。VASFA评分临床疗效优69例(90.8%),可7例(9.2%); 术后MRI检查提示修复的关节面平滑,移植物与周围组织结合良好。结论:自体距骨软骨移植治疗Hepple Ⅲ~Ⅴ型距骨软骨损伤能够缓解疼痛和改善踝关节功能,手术是胫骨截骨和距骨供区取骨、受区植骨移植同时进行的,未见取骨处并发症。
