摘要:
目的:观察通络生骨方治疗股骨头骨髓水肿综合征的临床疗效,为股骨头骨髓水肿综合征的治疗提供一种可行、有效的方法。方法:2016年9月至2018年9月,采用前瞻性配对比较方法,对100例股骨头骨髓水肿综合征患者进行研究。其中治疗组50例,采用通络生骨方治疗; 对照组50例,给予体外冲击波治疗。记录两组患者治疗前Harris评分、VAS评分及MRI水肿面积。治疗后定期随访,记录两组患者治疗后第2周、第4周、第8周及第12周Harris评分和VAS评分,并根据治疗后第12周MRI测量水肿面积。结果:100例患者脱落3例,剩下97例均获得随访。治疗后治疗组总有效率为93.88%,高于对照组的81.25%,差异有统计学意义(P<0.05); 两组患者治疗后Harris评分和VAS评分与治疗前比较,差异有统计学意义(P<0.05); 治疗后治疗组各时间段疼痛、功能、关节活动指数、总分分值均高于对照组,差异有统计学意义(P<0.05); 治疗组与对照组治疗后各时间段比较,治疗组分值明显低于对照组,差异有统计学意义(P<0.05); 两组患者治疗前后骨髓水肿面积(BME)比较,治疗后两组患者较治疗前水肿面积明显减小,差异有统计学意义(P<0.05); 治疗后治疗组水肿面积分值明显低于对照组,差异有统计学意义(P<0.05)。结论:通络生骨方在治疗上可缩短治疗时间,且通络生骨方在改善髋关节功能、缓解髋部疼痛及消除水肿面积方面略优于体外冲击波治疗。
Abstract:
To observe the clinical effect of Tongluo Shenggu recipe on femoral head of bone marrow edema(BME)syndrome,and to provide a feasible and effective method for the treatment of femoral head of bone marrow edema syndrome.Methods:From September 2016 to September 2018,100 patients with femoral head bone marrow edema syndrome were studied by prospective matched comparison method.Among them,50 cases in the treatment group were treated with Tongluo Shenggu decoction and 50 cases in the control group were treated with extracorporeal shock wave.Harris score,VAS score and MRI edema area were recorded before treatment in both groups.Harris score and VAS score were recorded at the 2nd,4th,8th and 12th week after treatment,and edema area was measured by MRI at the 12th week after treatment.Results:Three of the 100 patients dropped out,and the remaining 97 cases were followed up.The total effective rate of the treatment group was 93.88% after treatment,which was higher than that of the control group(81.25%),the difference was statistically significant(P<0.05),the Harris score and VAS score of the two groups after treatment were statistically significant(P<0.05),the pain,function,joint activity index and total score of the treatment group were higher than those of the control group at all time points after treatment(P<0.05),and the difference was statistically significant(P<0.05).After treatment,the score of the treatment group was significantly lower than that of the control group(P<0.05),BME of the two groups before and after treatment was significantly lower than that of the control group(P<0.05),after treatment,the edema area of the two groups was significantly smaller than that before treatment,and the difference was statistically significant(P<0.05),after treatment,the score of the edema area of the treatment group was significantly lower than that of the control group,the difference was significant.There was statistical significance(P<0.05).Conclusion:Tongluo Shenggu recipe can shorten the treatment time,and Tongluo Shenggu recipe is slightly better than extracorporeal shock wave in improving hip joint function,relieving hip pain and eliminating edema area.
