摘要:
目的:观察儿童股骨粗隆部骨肿瘤或瘤样病损并病理性骨折的手术疗效。方法:2012年3月至2016年3月治疗儿童股骨粗隆部骨肿瘤或瘤样病损并病理性骨折6例:男2例,女4例; 年龄8～12岁,平均10.3岁。病理结果:动脉瘤样骨囊肿3例,骨巨细胞瘤1例,骨纤维结构不良1例,孤立性骨囊肿1例。手术采取病灶刮除、同种异体骨植骨联合接骨板内固定。结果:6例均随访至恢复日常生活,随访时间12～40个月,平均26个月。参照骨与软组织肿瘤协会(MSTS)进行功能评估及Harris功能评分。术后最后复查股骨正侧位及髋关节正侧位X线片,病灶未见复发或转移,植骨区域骨愈合,内固定物无松动及断裂、变形等,手术部位均无感染、骨髓炎,双下肢等长,无再次骨折及髋内翻畸形等发生。结论:儿童股骨粗隆部骨肿瘤或瘤样病损并病理性骨折虽治疗困难,但手术中能利用多种手段处理瘤体细胞,采用同种异体骨充分植骨结合PHILOS接骨板的有效固定,可以减少复发,逐步恢复髋关节功能。
