摘要:
目的:观察正骨伸筋胶囊联合血小板裂解液对兔膝骨关节炎软骨组织形态及血清IL-1 和TNF-α含量的影响,初步明确正骨伸筋胶囊联合血小板裂解液治疗膝骨关节炎(KOA)的作用机制。方法:将6月龄30只新西兰大白兔随机分为对照组、氨基葡萄糖组和正骨伸筋胶囊联合血小板裂解液组,每组10只。分组后建立自发性KOA 模型,取耳缘静脉血。造模结束后,正常组、氨基葡萄糖组和正骨伸筋胶囊联合血小板裂解液组分别以纯水、盐酸氨基葡萄糖注射液、正骨伸筋胶囊注射液灌胃及血小板裂解液关节腔注射,1 次/d,连续干预2 个月后进行标本及血液采集。采集标本为新西兰大白兔右后下肢膝关节髌下韧带表面的滑膜和胫骨内侧平台软骨,制成石蜡切片; 血液取血为耳缘静脉血。软骨组织切片染色后光镜下观察软骨组织形态,采用Mankin's 病理评分标准进行评分; 对大白兔治疗前后血清IL-1 及TNF-α含量进行分析比较。结果:3组大白兔关节软骨Mankin's 评分比较,差异有统计学意义[(0.284±0.128)分,(3.790±0.480)分,(2.930±0.287)分,F=113.900,P<0.001]; 3组治疗前后血清IL-1 及TNF-α含量比较差异有统计学意义(P<0.05),治疗组治疗后血清IL-1 及TNF-α含量优于对照组,差异有统计学意义(P<0.05)。结论:正骨伸筋胶囊联合血小板裂解液和盐酸氨基葡萄糖均可延缓KOA 兔膝关节软骨退变,但正骨伸筋胶囊联合血小板裂解液的作用更加明显; 正骨伸筋胶囊能有效降低膝骨关节炎患者血清IL-1 及TNF-α含量,血小板裂解液可修复关节软骨,为其减少炎症刺激等作用机制提供了一定的分子细胞生物学及生物组织工程学方面的依据。
Abstract:
Objective:To observe the effect of Zhenggushenjin capsule combined with platelet lysate on cartilage morphology and content of serum IL-1 and TNF-α in rabbit knee osteoarthritis,and to clear the possible mechanism of Zhenggushenjin capsule combined with platelet lysate in the treatment of knee osteoarthritis initially.Methods:30 New Zealand white rabbits,6 months old,were randomly divided into the control group,the glucosamine group and Zhenggushenjin group,10 rats in each group.After grouping,the spontaneous KOA model was established and venous blood samples were collected from ear marginal vein.After the modeling,the normal group,the glucosamine group and Zhenggushenjin group were intragastrically administrated with pure water,Glucosamine hydrochloride solution,Zhenggushen solution respectively,and the latter received platelet lysate injection intraarticularly additionally,1 times a day,and the specimens and blood were collected 2 months later.The specimens were made from paraffin sections of synovial membrane and tibial medial platform cartilage on the surface of the inferior patellar ligament of the right hind leg of New Zealand white rabbits.Blood was taken from ear marginal veins.The cartilage morphology was observed under the light microscope after the cartilage sections were stained,and the score of Mankin 's pathological score was evaluated.The content of serum IL-1 and TNF-α was analyzed and compared before and after the treatment of the rabbits.Results:The Mankin's scores of articular cartilage in 3 groups were statistically significant [(0.284+0.128),(3.790+0.480),(2.930+0.287),F=113.900,P<0.001].In the three groups,there was a significant difference in the content of serum IL-1 and TNF-α before and after treatment(P<0.05),and the content of serum IL-1 and TNF-α in the treatment group was better than that of the control group(P<0.05).Conclusion:Zhenggushenjin capsule combined with platelet lysate and glucosamine hydrochloride both can delay the articular cartilage degeneration of KOA rabbit,but Zhenggushenjin capsule combined with platelet lysate effect is more apparent.Zhenggushenjin capsule can effectively reduce serum IL-1 and serum TNF- content in patients with knee osteoarthritis,and platelet lysate can repair articular cartilage.It provides some basis in molecular biology and biological tissue engineering for reducing inflammatory stimulation.
