摘要:
目的:探讨微型锁定加压钢板(LCP)结合拉力螺钉及克氏针治疗A型Lisfranc损伤的临床疗效,总结应用经验。方法:对27例A型Lisfranc损伤的患者进行微型LCP结合拉力螺钉及克氏针治疗,术后定期放射学检查并观察并发症情况,末次随访时依据美国足踝外科协会AOFAS评分系统对患者患足功能进行评估。结果:术后3~15个月(平均6.5个月)对27例患者进行随访,所有患者均未发生内固定物松动或断裂、骨折再次移位、骨折不愈合、伤口感染; 末次随访时3例出现行走疼痛,其中1例足弓形态异常,诊断为扁平足,2例出现创伤性骨关节炎表现,需口服镇痛药; 27例患者AOFAS评分结果示:优(90~100分)19例,良(80~89分)5例,可(70~79分)3例,足部功能恢复优良率88.9%.结论:微型LCP结合拉力螺钉及克氏针治疗A型Lisfranc损伤疗效显著,固定可靠,术后并发症少,是一种值得肯定的内固定方法。
