摘要:
目的:探讨前侧入路埋头加压螺钉内固定治疗前内侧面尺骨冠突骨折(O'Driscoll Ⅱ型)的临床疗效。方法:选取2013年1月至2014年12月采用前侧入路埋头加压螺钉内固定治疗的前内侧面尺骨冠突骨折患者10例,男7例,女3例; 年龄23～55岁,平均年龄40.5岁。根据尺骨冠突骨折O'Driscoll分型:O'Driscoll Ⅱa型者2例,O'Driscoll Ⅱb型者5例,O'Driscoll Ⅱc型者3例。术后随访观察骨折愈合时间、肘关节活动度及术后的Mayo肘关节功能评分情况。结果:术中5例患者加行外侧切口修补外侧副韧带,术后切口都一期愈合,未发现神经、血管损伤等手术并发症。10例患者术后随访11～28个月(平均21.7个月)。X线提示所有患者骨折均愈合,愈合时间为8～12周,平均10.4周。末次随访时肘部屈曲活动度120°～135°,平均115°; 肘部伸直活动度0°～11°,平均6.7°; 前臂旋前58°～76°,平均70.1°; 旋后73°～92°,平均81.2°.根据Mayo肘关节功能评分:优8例,良2例,中0例,差0例,优良率100%.随访期间无异位骨化及创伤性关节炎发生。结论:对于尺骨冠状突前内侧面骨折,采用肘关节前侧入路可清晰暴露骨折,埋头加压螺钉固定效果良好。
