摘要:
目的:评价口服八珍汤加减对单侧首次全膝关节术后血红蛋白降低的影响。方法:根据随机对照原则,选择2014年1月至2015年5月本科室行首次单侧人工全膝关节置换患者80例,随机分为A和B两组,两组患者均在全身麻醉下应用止血带的条件下进行手术。A组(试验组)患者术后6 h给予中药方剂八珍汤颗粒配方1剂,溶于200 mL清水中口服,术后每日1剂,分早晚2次服用,服用14 d,B组(对照组)患者术后6 h给予200 mL清水口服。记录术后引流量,术前、术后第1,7,14天血红蛋白数值,比较两组血红蛋白减少量。结果:两组患者术前血红蛋白及术后引流量无明显差异,术后血红蛋白均有不同程度的下降,A组较B组在术后第1天血红蛋白降低无明显差异(P>0.05),第7,14天血红蛋白减少量显著降低,差异有统计学意义(P<0.05)。结论:口服八珍汤加减能有效缓解首次单侧全膝关节置换术后血红蛋白降低。
Abstract:
Objective:To evaluate the effect of oral Bazhen Decoction on hemoglobin decrease during primary total knee replacement.Methods:From January 2014 to May 2015,80 cases underwent primary total knee replacement were randomly divided into group A and group B,and all the patients were treated with surgery under general anesthesia in the application of tourniquet.6 hours after operation,patients in group A take 1 dose of Bazhen decoction dissolved in 200 mL water,one dose daily, two times a day for 14 days.Patients in group B were given 200 mL water 6 hours after operation.The postoperative drainage volume,hemoglobin value before and 1,7,14 d after operation were recorded.Reduction of hemoglobin in the two groups was compared.Results:There was no significant difference in preoperative hemoglobin and postoperative drainage volume between the two groups.After the operation,the hemoglobin was decreased in different degrees,and there was no significant difference in hemoglobin decrease between the two groups in the first day after surgery(P>0.05).On the 7 and 14 days,hemoglobin reduction in A group was significantly lower,with statistical significance(P<0.05).Conclusion:Oral Bazhen decoction can reduce hemoglobin decrease during primary total knee replacement.
