摘要:
目的:观察应用打压植骨带缝匠肌蒂髂骨瓣移植治疗早中期非创伤性股骨头坏死的中远期疗效,探讨早中期股骨头坏死的保髋疗法。方法:回顾性选取2011年12月至2013年12月在本中心行打压植骨带缝匠肌蒂髂骨瓣移植的早中期非创伤性股骨头坏死患者51例(60髋),评估术前和末次随访的Harris评分、临床疗效、影像学评价及股骨头生存率。结果:51例患者中有3例失访,予以剔除,48例(56髋)获得随访,随访时间18～40个月,平均31.5个月; 末次随访总Harris评分(83.04±8.32)分,与术前相比明显提高,差异有统计学意义(P<0.01); 临床疗效评价总优良率80.36%; 末次随访股骨头总生存率78.57%,其中ARCO Ⅰ期生存率83.33%,Ⅱ期80.65%,Ⅲ期57.14%,经Log-rank检验差异有统计学意义(χ2=13.132,P<0.01)。结论:打压植骨带缝匠肌蒂髂骨瓣移植是治疗早中期股骨头坏死的有效手段,其中远期疗效满意,且ARCO Ⅰ期疗效较优。
Abstract:
Objective:To observe the long-term curative effect of sartorius muscle-pedicle bone graft for non-traumatic osteonecrosis of the femoral head(ONFH)in the early stage and discuss the treatment of hip-salvage operation for non-traumatic ONFH in the early stage.Methods:51 cases(60 hips)of non traumatic femoral head necrosis(HIPS)in the early and middle period in our center between December 2011 and December 2013 were selected,and sartorius muscle-pedicle bone graft was performed in the patients.Harris score,clinical efficacy,imaging evaluation and the survival rate of the femoral head were evaluated before operation and at the last follow-up.Results:3 patients were censored,the mean duration of follow-up was 31.5 months(range,18～40 months).As compared with pre-operation,the total Harris hip score at the last follow-up was improved(83.04±8.32),the difference was statistically significant(P<0.01).At the last follow-up,the efficacy evaluation showed that the total good rate was 80.36%,and the survival rate of femoral head was 78.57%.ARCO stage I survival rate was 83.33%,ARCO stage Ⅱ 80.65%,ARCO stage Ⅲ 57.14%,and the log rank test with statistically significant difference(χ2=13.132,P<0.01).Conclusion:Sartorius muscle-pedicle bone graft is an effective method for non-traumatic ONFH in the early stage.The long-term follow-up results are encouraging,and the curative effect is better in ARCO stage I.
