摘要:
目的:探讨并分析急性颈髓损伤患者并发低钠血症的高危因素,并针对高危因素提出防治对策。方法:2008年1月至2014年6月本院诊治369例急性颈脊髓损伤患者按有无并发低钠血症将研究对象分为两组:低钠血症组和对照组(正常血钠组),研究分析年龄、性别、高热、感染、颈髓损伤平面、损伤机制、治疗方法、饮食障碍、呼吸机使用等对急性颈髓损伤并发低钠血症发生的影响,采用Logistic多元回归分析及单因素分析相关因素产生的影响。结果:在相关的影响因素中单因素分析表明高热、感染、颈髓损伤平面和程度、呼吸机使用、饮食障碍对低钠血症是否发生具有显著影响,按OR值由大到小排列分别是:Frankel分级、损伤平面、感染、高热。结论:急性颈髓损伤并发低钠血症发病的独立影响因素主要有高热、感染、颈髓损伤平面和程度等,应及时干预相关因素,降低急性颈髓损伤并发低钠血症的发生率。
Abstract:
Objective:To detect and analysis the risk factors of patients with acute cervical spinal cord injury complicated with hyponatremia,and put forward the prevention measures for the high risk factors.Methods:All 369 patients with acute cervical spinal cord injury data were retrospectively analyzed and divided into two groups according to whether complicated with hyponatremia from January 2008 to June 2014.The multivariate Logistic regression analysis and univariate analysis were used to analyzed the risk factors,such as age,gender,high fever,infection,cervical spinal cord injury plane,mechanism of injury,treatment method,eating disorders,and ventilator use effect on the occurrence of hyponatremia after acute cervical spinal cord injury.Results:The related influence factors in univariate analysis showed that the fever,infection,cervical spinal cord injury plane and degree,ventilator use,and eating disorder were significantly impact on the occurrence of hyponatremia.According to the OR value from large to small are:Frankel classification,injury plane,infection,and high fever.Conclusion:The fever,infection,and cervical spinal cord injury plane and degree are independent risk factors of patients with acute cervical spinal cord injury complicated with hyponatremia.The early treatment should be taken to reduce the incidence of hyponatremia in patients of acute cervical cord injury.
