摘要:
目的:比较开窗植骨与开门植骨治疗胫骨平台骨折的临床疗效。方法:回顾性分析2012年2月至2014年6月以两种方法治疗胫骨平台骨折资料完整患者43例,其中开门组23例,开窗组20例,平均年龄44岁(34~63岁)。通过对临床和影像学资料进行分析,对比胫骨平台关节软骨面的复位及术后1年的软骨面再塌陷情况,膝关节功能恢复等情况。结果:全部患者均获得随访,平均随访16个月(12~30个月),均获得骨性愈合。开门组术后3天及1年后软骨面塌陷改善情况较开窗组好,差异有统计学意义(P<0.05)。术后3个月及1年的膝关节功能评分,开门组较开窗组优,差异有统计学意义(P<0.05)。结论:开门植骨治疗胫骨平台骨折较传统开窗植骨有更好的效果。
Abstract:
Objective:To compare the clinical efficacy of bone grafting in door and bone grafting in window for tibial plateau fractures.Methods:Retrospective analysis of 43 cases of tibial plateau fractures from February 2012 to June 2014.There were 23 cases treated with opening door and 23 cases treated with opening window.The mean age was 44 years old(34 to 63).The clinical and radiological imaging data were analyzed.The reduction of articular cartilage surface of the tibial plateau,the ratio of collapse of tibial plateau one year after surgery,and the knee function were compared in the two groups.Results:All patients were followed up for 12 to 30 months,for an average of 16 months.All patients were obtained complete union.The improvement of articular cartilage surface subsidence in 3 days and 1 year after surgery in opening door group was better than opening window group(P<0.05).The knee function score in 3 months and 1 year after surgery in opening door group was better than opening window group(P<0.05).Conclusions:The use of bone grafting in door is effective than bone grafting in window for tibial plateau fractures.
